URINE 10S STRIP URINE 10S STRIP URN-10100 Packing Size/ Ambalaj 1x 100 test : 2-30 °C GİRİŞ İdrarda Kan, Bilirubin, Ürobilinojen, Keton, Protein, Nitrit, Glukoz, pH, Dansite,ve Lökositin kalitatif ve yarı kantitatif tanısına uygun test stribi. UYARILAR VE ÖNLEMLER İdrar stripleri yalnız vücut dışı kullanım içindir. İdrar striplerinin üzerindeki test bölgelerine dokunmayınız. İdrar test stripleri hem diazonyum tuz hem de nitroferrisiyanid içerebilir. Cilt ve mukoz membranlara temasından kaçınınız; temas edilen alanları bol su ile yıkayınız. ÖZET İdrar stripi idrar numuneleri kullanarak vucüt dışı kullanım için geliştirilmiştir. Reaktif ilavesine yada laboratuvar ekipmanına ihtiyaç yoktur.Reaktif stripleri kurutucu madde (nem çekici) içeren plastik şişele yerleştirilir. Test striplerinin kapakları sıkıca kapatılmalıdır. Optimal sonuçlar için taze, iyi karıştırılmış ve santrifüjlenmemiş idrar kullanılması gerekmektedir. PROSEDÜRÜN KİMYASAL PRENSİPLERİ Kan: Bu test O-Tolidin ve tamponlanmış organik peroksit, kümen hidroperoksit reaksiyonunu katalizleyen hemoglobinin psödoperoksidaz aktivitesi üzerine kuruludur. Sonuçlanan renkler sarı-yeşilden koyu yeşile kadar değişir. Bilirubin: Bu test kuvvetli asit ortamda bilirubinin 2,4-diklorobenzen diazonyum, Na ile birleştirilmesi üzerine kuruludur. Renk skalası açık kahverengiden kızıl-kahverengine kadar uzanır. Ürobilinojen: Bu test üzerinde 4-metoksibenzen diazonyum tuzu ve idrar ürobilinojenin, kuvvetli asit ortamda deazotinasyon reaksiyonu üzerine kuruludur. Renkler açık pembeden parlak koyu kırmızıya doğru değişir. rengini maviden yeşil-kahveye, kahverengi ve koyu kahverengine kadar uzanan renk skalasında değiştirir. Keton: Bu test kuvvetli bazik ortamda asetoasetik asit ile sodyum nitroprussidin reaksiyonu üzerine kuruludur. Renkler negatif okumada bej ya da soluk pembe renginden pozitif okumada pembe-mora kadar değişir. Protein: Bu test protein varlığında tetrabromofenol mavi indikatörünün renk değişimini esas almaktadır. Pozitif reaksiyon sarıdan yeşile ve daha sonra mavi-yeşile renk değişimiyle gösterilir. Nitrit: Bu test idrardaki bakterilerin varlığında besinsel nitratın p-arsanilik asit ve nitrite dönüşümü üzerine kuruludur. Diazonyum bileşeni asidik ortamda N-(1-naftil)etilendiamin 2HCl ile eşleşir. Oluşan renk pembedir. Pembe rengin her derecesi pozitif olarak değerlendirilir. Glukoz: Test çift zamanlı enzim reaksiyonu üzerine kuruludur. İlk olarak, Glukoz oksidaz ,glukozun glukonik asit ve hidrojen peroksite oksidasyonunu katalizler. İkinci enzim Peroksidaz ise, oluşan hidrojen peroksitin potasyum iyodürle birlikte bir kromojenin oksidasyonunu katalizleyerek, kromojenin pH: Bu test bromtimol mavi ve metil kırmızısının tüm idrar pH aralığını kapsayan geniş renk aralığı veren bir çift pH belirleyici metodudur. Renkler turuncudan sarı-yeşile ve yeşilden maviye kadar değişir. Dansite: Bu test iyonik konsantrasyonlardaki polielektrolitlerin görünür pKa değişimleri üzerine kuruludur. Bir indikatörün varolması durumunda; düşük iyonik konsantrasyonlardaki idrarda renkler koyu mavi yada mavi-yeşilden , yüksek iyonik konsantrasyonlardaki idrarda yeşil ve sarı-yeşile kadar değişir. Lökosit: Bu test granülosit esterazların varlığını gösterir. Esterazlar hidroksi tiazol türevini oluşturmak için tiazol aminoasit ester türevlerini keser. Bu tiazol daha sonra mor renk üretebilmek için diazonyum tuzuyla reaksiyona girer. Stripleri buzdolabi yada derin dondurucuda saklamayınız. Test stripleri nem,sıcaklık ve ışık gibi spesifik çevresel faktörlere karşı hassas olduğundan dolayı stripleri bu faktörlere maruz bırakmayınız. TAVSİYE EDİLEN TAŞIMA PROSEDÜRLERİ Tüm kullanılmamış stripler orijinal şişesinde saklanmalıdır. Kurutucuyu(nem alıcı) şişeden çıkarmayınız. Striplerin başka bir şişeye transferi reaktif striplerin bozulmasına ve non-reaktif olmasına neden olabilir. Test striplerini çıkardıktan sonra kapağı sıkıca kapatın. Striplerin test alanına dokunmayınız. Son kullanma tarihi geçen stripleri kullanmayınız. Çalışma alanı temiz olmalı, deterjan ve diğer kontaminantlardan arındırılmış olmalıdır.. ÖRNEK TOPLAMASI VE HAZIRLIĞI İdrarı temiz, kuru bir kabın içinde toplayın ve en kısa zamanda test edin. Eğer test bir saat içinde çalışılmayacaksa örneği hemen soğutun.Test etmeden önce soğutulan örneğin oda sıcaklığına gelmesini bekleyin. TEST PROSEDÜRÜ 1. Ürünün etiket üzerindeki son kullanma tarihini kontrol ediniz. 2. Şişeden hemen kullanım için sadece yeterli miktarda stripi alın ve kapağı sıkıca kapatın. 3. Stripi kontrol ediniz. Reaktif bölgelerinin rengini yitirmesi veya kararması bozulmayı gösterebilir. Stripi kullanmayınız. 4. Stripin reaktif bölgelerini taze iyi karıştırılmış idrarın içine 1 saniyeden fazla olmayacak süreyle tamamen batırın. Çıkarırken , fazla idrarı almak için idrar kabının kenarına değil stribin kenarına dokunun. Stribin uzun kenarını emici bir kağıt havluyla kalan idrarı iyice temizlemek için ve akmasını engellemek için silin. 5. Her bir reaktif bölgesini renk skalasındaki uygun renk sütunuyla karşılaştırın. Optimal sonuç için doğru zamanda okumak (30-60 sn) önemlidir. Kimyasalların etkileşiminden kaçınmak için stripi yatay pozisyonda tutunuz. Not:Test alanının sadece uçlarından görülen renk değişiklikleri yada 2 dakikadan sonra oluşan renk değişimlerinin diagnostik değeri yoktur. KALİTE KONTROL Stripler düzgünce saklanmalıdır ve test öncesince ve sürecinde hazırlanmalıdır. Reaktif striplerinin performansları için en iyi sonuç , bilinen negatif ve pozitif örnekleri test ederek ya da test edilen analitlerin her birinin normal yada abnormal miktarlarını içeren çoklu idrar kontrolleri ile onaylanmalıdır. SONUÇLAR Sonuçlar şişenin üzerinde basılı olan renk sütunlarının direkt karşılaştırmasıyla elde edilir. PROSEDÜR LİMİTLERİ Azo boyaları, nitrofurantoinve riboflavin içeren ilaçlar gibi anormal idrar rengine sebep olan maddeler idrar analiz reaktif striplerindeki reaktif alanlarının okunurluğunu etkileyebilir. Reaktif pedi üzerindeki renk oluşumu maskelenebilir yada görsel veya cihazla yalancı pozitif olarak yorumlanabilir. Bu yüzden şüphe varsa test ilaç tedavisi bırakıldığında tekrar edilmelidir. Kan: Kan testinin hassasiyeti idrarda protein ve/veya yüksek askorbik asit içeriğiyle azalır. Hipoklorid gibi yüksek oksitleyici maddeler yalancı pozitif sonuç verebilir. Menstrüasyon dönemindeki kadınların idrarında sıklıkla pozitif sonuç çıkabilir. Bilirubin: Normalde en hassas metodlarla bile idrarda bilirubin belirlenemez. Numunedeki bilirubin ışığa karşı hassas olduğu için idrar numunelerinin uzun süreli ışığa maruz kalması yalancı negatif sonuç verdirebilir. 25-50 mg/dL askorbik asid konsantrasyonu yanlış negatif sonuca sebep olabilir.Yalancı pozitif sonuçlar test idrarında diagnostik yada teröpatik boyaların varlığıyla ortaya çıkabilir. Ürobilinojen: Ürobilinojenin yokluğu bu testle tespit edilemez. Normal idrar numuneleri uçuk pembe renk verir.Formalinin yüksek konsantrasyonu yalancı negatif sonuç verebilir. Bu test porfobilinojen bulgusu için güvenilir bir metod değildir Keton: Normal idrar numuneleri bu reaktifle negatif sonuç verir. Yalancı pozitif sonuçlar yüksek pigmentli idrar numuneleriyle yada yüksek miktarda L-dopa metaboliltlerinin bulunduğu idrar örneklerinden sağlanabilir.2 Protein: İdrarda bu testin minimum hassasiyeti 5-10 mg/dL proteindir. Yüksek tamponlu alkali idrarlar yalancı pozitif sonuç verebilir.2 Bulanık idrar numunelerinde sonuçların yorumlanması zordur. Nitrit: Pembe noktalar yada pembe uçlar pozif olarak değerlendirilmemesine rağmen tek bir pembe renk oluşunun herhangibir derecesi pozitif olarak düşünülmelidir. Renk oluşumu, bulunan bakteri miktarıyla orantılı değildir. Nitrit testi sadece nitratı indirgeyen bakterileri belirler. Ara sıra nitratı nitrite çevirmeyen bakteriler bulunabilecektir. Bu yüzden bu test sabah alınan ilk idrar için spesifiktir 2. Nitrit testinin hassasiyeti yüksek dansite yada 25 mg/dL den fazla olan askorbik asit ile azaltılır. Glukoz: Glukoz testinin reaktivitesi Dansite ve/veya idrarda askorbik asid miktarı arttıkça azalır ve aynı zamanda sıcaklığa bağlı olarak da değişebilir. Askorbik asit (50mg/dL yada fazlası) ve keton cisimlerinin ortalama miktarları (40mg/dL yada fazlası), az miktarda glukoz bulunan idrarda(100 mg/dL) renk gelişimini azaltabilir. Ancak, keton ile aynı anda glukoz konsantrasyonunu metabolik olarak izlemek imkansızdır. pH: Bu test için pH aralığı 5-9 dur. Hipertansiyon ve kalp problemlerinde kullanılan ilaçlar alkali idrara sebep olabilir2. Fazla idrar stripte kalırsa "kontaminasyon" olarak bilinen durum ortaya çıkar, protein reaktif bölgesinden pH bölgesine geçen asit tamponu pH sonucunda yanlış bir azalmaya neden olur. Dansite: Yüksek dansite okumaları ölçülü miktarlardaki proteinin (100-750 mg/dl) varlığında sağlanabilir. Dansite idrardaki glukozla azalır. Lökosit: Bu test sonucu mikroskobik incelemedeki lökosit sayısıyla tutarlı olmayabilir5.Yüksek nem ve yüksek sıcaklık koşullarında ve şişe emniyetinin başarısız olduğu durumlarda yalancı pozitif sonuçlar ortaya çıkabilir. Yüksek glukoz konsantrasyonu, yüksek dansite, yüksek seviye albumin , yüksek konsantrasyon formaldehit yada kanın varlığı azalan sonuçlara neden olabilir. Oksalik asit ve oksitleyici ajanların yüksek konsantrasyonu yalancı negatif sonuç ortaya çıkarabilir. Düşük dansite (1.010 ve aşağısı) yalancı pozitif sonuca sebep olabilir. BEKLENEN DEĞERLER Kan:İdrarda hemoglobin görülmesi bazı böbrek hastalıklarını yada üriner sistem bozukluklarını gösterir. Bu testin pratik belirleme limiti her mikrolitre idrarda yaklaşık 5-10 eritrosittir. Bu test hemoglobine oldukça hassastır . Bilirubln:.Normalde en duyarlı testle bile, daha fazla inceleme gerektirecek kadar abnormal olan idrardaki bilirubin bulunamaz. Atipik renkler ( renk skalasında görünen negatif ya da pozitif renk sütunlarından farklı oluşan renkler) idrar numunesinde mevcut olan bilirubin kaynaklı safrayı işaret eder. “+” (0.5 mg/dL), “++” (1.0 mg/dL), “+++” (3.0 mg/dL) konsantrasyonları karaciğer hasarı veya safra tıkanmasını işaret eder. Ürobilinojen: Sağlıklı bir popülasyonda, bu testle sağlanan normal idrar ürobilinojen aralığı 0.1-1.0 mg/dL dir (1 mg/dL yaklaşık 1 Ehrlich Unit/dL ye eşittir). 2.0 EU/dl lik bir sonuç klinik öneme sahip olabilirve aynı hastanın numunesi daha fazla incelenmelidir Keton: Normalde idrarda keton mevcut değildir. ”± “ (5 mg/dL), “+” (15 mg/dL), “++” (40 mg/dL), “+++” (80 mg/dL) konsantrasyonları idrardaki asetik asit konsantrasyonuyla uyum göstermektedir. İdrarda bulunabilen keton seviyeleri hamilelik, diyet, sık ve sert egzersizler gibi psikolojik stres koşullarında ortaya çıkar. Protein: Normal idrar numuneleri az protein (0-4 mg/dL) içerir. Bu yüzden bu sınırların aşılması böbrek yada üriner sistem hastalığını işaret eder. “+” (30 mg/dL), “++” (100 mg/dL), “+++” (300 mg/dL) , “++++” (1000 mg/dL) konsantrasyonlar idrardaki albumin konsantrasyonuyla uyum gösterir. Patolojik proteinüri genellikle 30 mg/dL nin üzerinde sonuç verir ve kalıcıdır. Nitrit:Nitrit için idrarı test etme idrardaki bakteri testi içindir. 30 saniye sonra pembe rengin herhangibir derecesi klinik olarak belirgin bakteriüriyi gösterir. Bakteriüri genellikle böbrek, üreter enfeksiyonlarına bağlıdır. Glukoz: Normal olarak, küçük miktarda glukoz vücuttan böbrekle atılmaktadır. Bu test stripiyle yaklaşık 50-100 mg/dL glukoz idrarda belirlenebilir. 100 mg/dL konsantrasyon anormal olarak değerlendirilebilir. pH: idrar pH’sı genellikle 5-8 dir. İdrar pH sı metabolik,böbrek, gastrointestinal ve solunum faktörlerinin önemli indikatörüdür. Dansite: Sıradan idrar örneklerinde dansite 1.003-1.040’ ye kadar değişir. Normal beslenen ve normal sıvı alan normal yetişkinlerden alınan yirmidört saatlik idrarın dansitesi 1.016-1.022 olacaktır. Şiddetli böbrek hasarında dansite 1.000-1.030 arasıdır. Lökosit: Normal idrar örnekleri genelde bu testle negatif sonuç verir. Bireysel olarak az miktarda bulunan sonuçların kuşkulu klinik önemi vardır. SPESİFİK PERFORMANS KARAKTERİSTİKLERİ İdrar striplerinin performans karakteristikleri hem laboratuar hem de klinik testlerde belirlenmiştir.Kullanıcının önem parametreleri hassasiyet, özgünlük, doğruluk ve kesinliktir. Genelde idrar stripleri yukarıda listelenen interferanlar dışında ölçülen bileşenler için spesifik olarak geliştirilmiştir. Görsel okunan stripler için, kesinlik, şişe etiketinin üzerindeki renk bloklarının belirlendiği ve okuma testinde insan gözünün ince ayrımları farkedebilmesi davranışının fonksiyonudur. Bu tür bir testte insan gözünün çeşitliliğinden dolayı kesinliği değerlendirmek güçtür. Bu yüzden kullanıcıları kendi performans standartlarını geliştirme konusunda teşvik edilir. Kan: Bu test serbest hemoglobin ve myoglobine karşı tüm eritrositlere olduğundan çok daha hassastır. Bu testin belirtildiği şekilde okunduğunda hemoglobinden yoksun 0.015 mg/dL yada 5-10 tam eritrosit hücreleri/µL idrar hassasiyeti vardır. Hassasiyet idrarda yüksek dansite ve askorbik asit içeriğiyle azalabilir. Reaktif test alanında yeşil noktaların görünümü idrarda eritrosit varlığını gösterir. Bilirubin: Bu test idrardaki bilirubinde 0.5 mg/dL lik bir hassasiyete sahiptir. İdrardaki bilirubin klinik belirtilerden önce karaciğer hastalığını gösterebilir. Urobilinojen: Bu test idrardaki 0.1 EU/dL kadar az konsantrasyondaki ürobilinojeni bulur. Bu yüzden çoğu normal idrar açık pembe reaksiyon verebilir. Keton: Keton test bölgesi yarı- kantitatif sonuçlar verir ve idrardaki asetoasetik asitle etkileşim halindedir.Bu test beta-hidroksibutirik asit yada asetonla reaksiyona girmez. Reaktif bölgesi 100 mL idrardaki 5mg/dL asetoasetik asidi belirler. Bazı yüksek dansite ve düşük pH’lı idrarlar 5 mg/dL ye kadar olan reaksiyonları verebilirler. Protein: Test bölgesi albümine globulin, hemoglobin, Bence-Jones proteinleri, ve mukoproteinden daha duyarlıdır. Nitrit: Bu test normal dansite ve uygun askorbik asit seviyelerinde idrarda 0.05 mg/dl nitrit iyonu ve yaklaşık 105/dL bakteri hassasiyetine sahiptir. Bu test nitrit için spesifiktir ve normalde idrarla dışarı atılan maddelerle reaksiyona geçmez. Glukoz: Bu test glukoz için spesifiktir. İdrardaki yaklaşık 100 mg/dL glukoz belirlenebilir. Glukoz dışında idrarla atılan maddelerin pozitif sonuç verdiği bilinmemektedir. Levadopa, askorbik asit, glutatyon ve dipironun varlığında yalancı negatif sonuç verebilir. pH: Bu test pH 5-9 değerlerinin üzerinde turuncudan maviye kadar değişen renkler oluşturur. Bununla birlikte idrardaki pigmentlerin neden olduğu önemsenmeyecek varyasyonlar yüzünden doğru okuma bozulabilse de değerler 1 ünite farklılık gösterecektir. Dansite: Spesifik dansite testi 1.000 ila 1.030 arasında idrar dansitesinin belirlemesine izin verir. Yüksek tamponlu alkali idrarlar sonuçların düşük okunmasına sebep olabilir. WARNINGS AND PRECAUTIONS Urine Strips are for in vitro diagnostic use. Do not touch test areas of Urine Strips. Urine test strips may contain either diazonium salt or nitroferricyanide. Avoid contact with skin and mucous membranes; flush affected areas with copious amounts of water. SUMMARY Urine strip is intended for use as an in vitro diagnostic aid using urine specimens. The strip contains solid phase reagent areas affixed to a plastic support and is provided in a dry reagent format. This strip provides tests for, qualitatively and semiquantitatively, urobilinogen, glucose, ketones,bilirubin, protein, nitrite,pH,occult blood, specific gravity and leukocytes by visual comparison with color charts of each concentration range. No additional reagents or laboratory equipment is required.The reagent strips are packaged in a plastic vial containing desiccant. The test strips must be tightly capped. It is necessary to use fresh,well mixed and unsantrifuged urine for optimal results. CHEMİCAL PRlNCIPLES OF THE PROCEDURE Blood: This test is based on the pseudoperoxidase activity of hemoglobin which catalyzes the reaction of O-Tolidine and buffered organic peroxide, Cumene hydroperoxide. The resulting colors range from yellow-green through dark green. Bilirubin: This test is based on the coupling of bilirubin with 2,4-dichlorobenzene diazonium Na in a strongly acid medium. The colors range from light tan to purple. Urobilinogen: The test is based on the diazotization reaction of 4-methoxybenzene diazonium salt and urinary urobilinojen in a strong acid medium. The color changes range from pink to brown-red. Ketones: This test is based on the reaction of acetoacetic acid in the urine with nitroprusside. The resulting color ranges from tan when no reaction takes place to purple for positive reaction. Protein: This test is based on the color change of the indicator,tetrabromophenol blue,in the presence of the protein. A positive reaction is indicated by a color change from yellow through greenn and then greenish-blue. Nitrite: This test is based on the reaction of p-arsanilic acid and nitrite (which is derived from dietary nitrate in the presence of bacteria) in urine to form a diazonium compound. The diazonium compound in turns couple with N-(1-naphthyl) ethylenediamine 2HCl in an asidic medium. The resulting color is pink. Any degree of pink color is considered positive. Glucose: This test is based on a sequential enzyme reaction. First, glucose oxidase, catalyzes the formation of gluconic acid and hydrogen peroxide from the oxidation of glucose. A second enzyme, peroxidase, catalyzes the reaction of hydrogen peroxide with potassium iodide chromogen to oxidize the chromogen to colors ranging from blue-green to greenish-brown through brown and dark brown. pH: This test is based on double indicators ( bromothymol blue and methyl red) which give a broad range of colors covering the entire urinary pH range Colors range from orange through greenish yellow and green to blue. Specific gravity: This test is based on the pKa change of certain pretreated polyelectrolytes in relation to ionic concentration. In the presence of an indicator, color range from deep blue in urine of low ionic concentration through green and yellow-green in urines of increasing ionic concentration. Leukocytes: This test reveals the presence of granulocyte esterases. The esterases cleave an derivatized thiazole aminoacid ester to liberate derivatized hydroxy thiazole. This thiazole then reacts with a diazonium salt to produce a purple product. Do not store the strips in the refrigerator or freezer.Since test strips are sensitive to specific enviromental factors,such as moisture,heat and light, do not expose strips to these factors. RECOMMENDED HANDLING PROCEDURES All unused strips must remain in the original bottle. Do not remove desiccant from bottle. Transfer of the strips to another container may cause reagent strips to deteriorate and become unreactive. After taking out the test strips, replace the cap promptly and tightly.Do not touch the test area of the strip. Do not use strips after expiration date. The work area should be clean and free of detergents and other contaminants. SPECIMEN COLLECTION AND PREPARATION Use a clean,dry,unused vessel to collect the urine. Test the urine as soon as possible after collection. If testing can not be done within an hour after voiding,refrigerate the specimen immediately and let it return to room temperature before testing. TEST PROCEDURE 1. Confirm that the product is within the expiration date shown on the label. 2. Remove from the bottle only enough strips for immediate use and replace cap tightly. 3. Inspect the strip. Discoloration or darkening of reagent areas may indicate deterioration. Do not use the strip. 4. Dip the test strip completely for no more than 1 second in fresh,well-mixed and uncentrifuged urine specimen. Excessive urine on the test strip may give rise to a wrong result. Remove the excessive urine by touching the plastic film on the rim of vessel. Excessive urine may be removed by gently blotting the lenghtwise edge on absorbent paper. 5. Compare the test results carefully with the color chart on the bottle label in good light.Proper reading time (30-60 seconds) is critical for optimal results. While comparing,keep the strip in horizontal position to avoid possible interaction of chemicals by excessive urine. Note: Changes in color that appear only along the edges of the test areas or after more than two minutes have passed are of no diagnostic significance. QUALITY CONTROL The strips must be properly stored and handled before and during the testing. Reaction of reagent strips should be confirmed by testing known positive and negative specimens or multiple analyte controls containing normal and abmormal amounts of each of the analytes being tested. RESULTS Results are obtained by direct comparison of test strip with the color blocks printed on the bottle label. No calculations or laboratory instruments are necessary. LlMITATIONS OF PROCEDURE Substances that cause abnormal urine color such as drugs containing azo dyes,nitrofurantoin and riboflavin may affect the readability of reagent areas on urinalysis reagent strips. The color development on the reagent pad may be masked, or a color reaction may be produced on the pad that could be interpreted visually and instrumentally as a false positive. It is therefore recommended that in case of doubt, the test should be repeated after withdrawal of the medication. Blood: A false positive can sometimes occur when bacteria are present in the urine. Ascorbic acid or protein may be reduce the reactivity of the blood test. Strong oxidizing substances,such as hypochlorites,may produce a false positive result. Urine from menstruating females often yields positive results. Bilirubin: Normally no bilirubin is detectable in urine by even the most sensitive methods. Since the bilirubin in specimens is sensitive to light, exposure of the urine specimens to light for a long period time may result in a false negative. Ascorbic acid concentration of 25-50 mg/dl may also cause false negatives. Even trace amounts of bilirubin are sufficiently abnormal to require further investigation. False positive results may be obtained in the presence of diagnostic or therapeutic dyes in the test urine. Urobilinogen: A complete absence of urobilinogen in the specimen being tested cannot be demonstrated by the strip. Normal urine specimens ordinarily give a slight pink color. High concentration of formalin may give false negative result. This test is not a reliable method for detection of prophobilinogen. Ketones: Normal urine specimens ordinarily yield negative results with this reagent. False positive results may occur with highly pigmented urine specimens or those containing large amounts of levodopa metabolites.2 Protein: The minimum sensitivity of this test is 5-10 mg/dl of protein in urine. Highly buffered alkaline urines (pH 9) may give a false positive results.2 The interpretation of results is also difficult in turbid urine specimens. Nitrite: Any degree of uniform pink color development should be considered positive,however,pink spots or pink edges should not be interpreted as a positive result. Color development is not proportional to the number of bacteria present. The nitrite tests detects only nitrate reducing bacteria. Occasionally bacretia will be present that do not reduce nitrate to nitrite. Therefore, a negetive,therefore, a mid stream of first morning urine specimen is recommended for this test.2 Sensitivity of the nitrite test is decreased with high specific gravity or ascorbic acid concentrations of 25 mg/dl or greater. Glucose: The reactivity of the test decreases as the specific gravity and/or pH of the urine increases. and may also vary with temperature. Ascorbic acid (more than 50 mg/dl) and ketone bodies (more than 40 mg/dl) may cause a false negative for a specimen containing a small amount of glucose (100 mg/dl), however, the combinations of such ketone levels and low glucose levels are metabolically improbable in screening. pH: This pH test indicates the pH values only within the range of 5 to 9. Certain drugs, such as those used for hypertension and heart trouble (Acetazolamides) may cause alkaline urine.2 Excessive urine on the test strip may wash the acid buffer from the neighboring protein reagent onto the pH area and change the pH reading to an acid pH although the urine being tested is originally neytral or alkaline. This is called the “runover” phenomenon. Specific gravity: Elevated specific gravity readings may be obtained in the presence of moderate quantities (100 mg-700 mg/dl) of protein,specific gravity is increased with the glucose in the urine. Leukocytes: The test result may not always be consistant with the leukocyte cell number by the micriscobic examination.5 Positive results may be found high humidity and high temperature condition, and failure of bottle security. High concentration of glucose, high specific gravity,high level of albumin,high concentration of formaldehyde, or presence of blood may caused decreased test results. High concentration of oxalic acid or trace of oxidizing agents may cause false negative results. Low specific gravity (1.010 and under) may cause a false positive. EXPECTED VALUES Blood: Hemolysis is a naturel process of recycling old or damaged red cells.But when hemoglobin appears in urine, it indicates some kind or utrinary tract disorder. The practical determination limit of this test is approximately 5 to 10 erythrocytes per microliter of urine.This test is highly sensitive to hemoglobin. Bilirubin: No bilirubin is detectable in urine of healty people by even the most sensitive methods. Elevated bilirubin in urine always indicates disease and is the earliest sign of liver cell disease and/or biliary obstruction. The signs of “+” (0.5 mg/dL), “++” (1.0 mg/dL), “+++” (3.0 mg/dL) signify THA qualitative severity of THA liver damage or bile obstruction. Urobilinogen: In this test stripthe normal urobilinogen range is 0.1 to 1.0 mg/dl (1 mg/dl is approximately equal to 1 Ehrlich unit/dl).3 If results exceed the concentration of 2.0 mg/dl, the patient and/or urine specimen should be evaluated further. Ketones: ketone bodies shoul not be detected in normal urine specimens with this reagent. The concentrations given: “±” 5 mg/dL), “+” (15 mg/dL), “++” (40 mg/dL), “+++” (80 mg/dL) correlate well with THA acetoacetic acid concentration in urine. THA sensitivity of this test is 5 mg acetoacetic acid per 100 ml of urine. Detectable levels of ketone may occur with frequent vomitting,diarrhea,digestive disturbances,pregnancy or severe physical exercise.4 Protein: Normal urine specimens ordinarily contain some protein (0-4 mg/dl);therefore,only persistent elevated levels of urine protein indicate kidney or urinary tract disease. The persistent results of trace level or over indicates significant proteinuria and thus further clinical testing is needed to evaluate the significance of results. The concentrations given; “+” (30 mg/dL), “++” (100 mg/dL), “+++” (300 mg/dL) , “++++” (1000 mg/dL) correlate well with the albumin concentrations in urine. Pathologic proteinuria generally gives values above 30 mg/dl and is persistent. Nitrite: Testing of urine for nitrite tests for bacteria in urine. Any degree of pink color after 30 seconds indicates clinically significant bacteriuria. Bacteriuria is generally due to infection of kidneys,ureters,bladder or urethra. Glucose: Normally no glucose is detectable in urine,although a minute quantity of glkucose is excreted by normal kidney. Approximately 50-100 mg glucose/dl of urine is detectable in this test strip. Concentration of 100 mg/dl may be considered as abnormal if found consistently. pH: Normal urine is slightly acid with a pH of 6, and urine pH values generallyrange from 5 to 8 pH units. The pH of urine is an important indicator of certain metabolic,kidney,gfastrointestinal and respiratory factors. Specific gravity: Adults random urines may be in specific gravity from 1.003 to 1.040, twenty four hour urines from normal adults with normal diets and normal fluid intake will have a specific gravity of 1.0161.022. This test permits determination of urine SG between 1.000 and 1.030. Leukocytes: Normally no leukocytes are detectable in urine. Individually observed trace results may be questionable clinical sişgnificance. SPECIFIC PERFORMANCE CHARACTERlSTICS The performance characteristics of Urine Strips have been determined both in the laboratory and in clinical tests. Parameters of importance to the user are sensitivity, specificity, accuracy, and precision. Generally, Urine Strips have been developed to be specific for the constituent to be measured with the exception of interferences listed above. For visually read strips, accuracy is a function of the manner in which the color blocks on the bottle label are determined and the discrimination of the human eye in reading the test. Precision is difficult to assess in a test of this type because of the variability of the human eye. It is for this reason that users are encouraged to develop their own standards of performance.The stability test has been developed by statistical procedure for various enviromental conditions. Blood:This test is slightly more sensitive to free hemoglobin and myoglobin than to intact erythrocytes.The test is generally capable of detecting 0.015 mg/dl free hemoglobin or 5-10 intact red blood cells per microliter urine. The sensitivity may be reduced in urines with high specific gravity and ascorbic acid content. The appearance of green spots on the reagent teat area indicates the presence of intact erythrocytes in the urine. Urobilinogen: This test can detect urobilinogen in concentrations as low as 0.1 EU/dl; therefore,most normal urines may give a slight pink reaction. Bilirubin: The test has a sensitivity of 0.5 mg/dl bilirubin in urine. Bilirubin in urine indicates liver disease before any clinical signs are usually evident. Ketone: The reaction of this reagent pad is caused by acetoacetic acid in urine, acetone or betahydroxybutyric acid makes no significant contribution to this test. Ordinarily, the reagent area detects 5.0 mg of acetic acid in 100 ml urine. Some high spesific gravity and low pH urines may give reactions up to and including trace level (5.0 mg/dl) Protein: The test is more sensitive to albumins than gamma-globulins,Bence-Jones proteins and mucoproteins; such proteins do not interfere with the reaction of albumin. Nitrite: This test has a sensitivity of 0.05 mg/dl nitrit ion or amount of bacteria about 105/dl in urine of normal spesific gravity and moderate levels of ascorbic acid. The test is specific for nitrite and independent of urinary pH of any other substance normally excreted in urine. Glucose: The test has a sensitivity of 50-100 mg glucose in 100 ml urine and is specific for glucose. No substance excreted in urine other than glucose is known to give a positive result. False negative results may be obtained with the presence of levodopa,ascorbic acid,glutathione and dipyrone. If the test color appears somewhat mottled at the higher glucose concentrations,match the darkest color to the color blocks. pH: This test produces distinct color changes from orange to blue over the pH value 5-9. Values will be read to within 1 unit; however an accurate reading may be confused because of slight variations caused by pigments in urine. Specific gravity:This test permits determination of urine specific gravity between 1.000,1.005,1.010, 1.015,1.020,1.025 and 1.030.Highly buffered alkaline urines may cause low reading of result.
