Değerlendirme süreci Dergiye gönderilen yazılar, ilk olarak dergi standartları açısından incelenir. Derginin istediği forma uymayan yazılar, daha ileri bir incelemeye gerek görülmeksizin yazarlarına iade edilir. Zaman ve emek kaybına yol açılmaması için, yazarlar dergi kurallarını dikkatli incelemeleri önerilir. Dergi kurallarına uygunluğuna karar verilen yazılar Editörler Kurulu tarafından incelenir ve en az biri başka kurumdan olmak üzere iki ya da daha fazla hakeme gönderilir. Yayınlanması için başvuruda bulunulan makalelerin, adı geçen tüm yazarlar tarafından onaylanmış olması ve çalışmanın başka bir yerde yayınlanmamış olması da yayınlanmak üzere ya değerlendirmede olmaması gerekmektedir. Gönderilen yazılar, yazım kuralları açısından Uluslararası Tıp Editörleri Komitesi tarafından hazırlanan “Biomedikal Dergilere Gönderilen Makalelerde Bulunması Gereken Standartlar “ a ( Uniform Requirements For Manuscripts Submittted to Biomedical Journals ) uygun olarak hazırlanmalıdır. Birimler, Ağırlıklar ve Ölçüler 11. Genel Konferansı'nda kabul edildiği şekilde Uluslararası Sistem (SI) ile uyumlu olmalıdır. Makaleler Word, WordPerfect, EPS, LaTeX, text, Postscript veya RTF formatında hazırlanmalı, şekil ve fotoğraflar ayrı dosyalar halinde TIFF, GIF, JPG, BMP, Postscript, veya EPS formatında kabul edilmektedir. Yazı kategorileri Yazının gönderildiği metin dosyasının içinde sırasıyla, Türkçe başlık, özet, anahtar sözcükler, İngilizce başlık, özet, İngilizce anahtar sözcükler, makalenin metini, kaynaklar, her sayfaya bir tablo olmak üzere tablolar ve son sayfada şekillerin (varsa) alt yazıları şeklinde olmalıdır. Metin dosyanızın içinde, yazar isimleri ve kurumlara ait bilgi, makalede kullanılan şekil ve resimler olmamalıdır. Özgün Araştırma Makaleleri Türkçe ve İngilizce özetler yazı başlığı ile birlikte verilmelidir. (i)özetler: Amaç (Objectives), Gereç ve Yöntem (Materials and Methods) ya da Hastalar ve Yöntemler (Patients and Methods), Bulgular (Results) ve Sonuç (Conclusion) bölümlerine ayrılmalı ve 200 sözcüğü geçmemelidir. (ii) Anahtar Sözcükler Index Medicus Medical Subject Headings (MeSH) ‘e uygun seçilmelidir. Yazının diğer bölümleri, (iii) Giriş, (iv) Gereç ve Yöntem / Hastalar ve Yöntemler, (v) Bulgular, (vi) Tartışma ve (vii) Kaynaklar'dır. Başlık sayfası dışında yazının hiçbir bölümünün ayrı sayfalarda başlatılması zorunluluğu yoktur. Maddi kaynak , çalışmayı destekleyen burslar, kuruluşlar, fonlar, metnin sonunda teşekkürler kısmında belirtilmelidir. Olgu sunumları İngilizce ve Türkçe özetleri kısa ve tek paragraflık olmalıdır. Olgu sunumu özetleri ağırlıklı olarak mutlaka olgu hakkında bilgileri içermektedir. Anahtar sözcüklerinden sonra giriş, olgu(lar) tartışma ve kaynaklar şeklinde düzenlenmelidir. Derleme yazıları İngilizce ve Türkçe başlık, İngilizce ve Türkçe özet ve İngilizce ve Türkçe anahtar kelimeler yer almalıdır. Kaynak sayısı 50 ile sınırlanması önerilmektedir. Kaynaklar Kaynaklar yazıda kullanılış sırasına göre numaralanmalıdır. Kaynaklarda verilen makale yazarlarının sayısı 6 dan fazla ise ilk 3 yazar belirtilmeli ve İngilizce kaynaklarda ilk 3 yazar isminden sonra “ et al.”, Türkçe kaynaklarda ise ilk 3 yazar isminden sonra “ ve ark. “ ibaresi kullanılmalıdır. Noktalamalara birden çok yazarlı bir çalışmayı tek yazar adıyla kısaltmamaya ve kaynak sayfalarının başlangıç ve bitimlerinin belirtilmesine dikkat edilmelidir. Etik Marmara Medical Journal’a yayınlanması amacı ile gönderilen yazılar Helsinki Bildirgesi, İyi Klinik Uygulamalar Kılavuzu,İyi Laboratuar Uygulamaları Kılavuzu esaslarına uymalıdır. Gerek insanlar gerekse hayvanlar açısından etik koşullara uygun olmayan yazılar yayınlanmak üzere kabul edilemez. Marmara Medical Journal, insanlar üzerinde yapılan araştırmaların önceden Araştırma Etik Kurulu tarafından onayının alınması şartını arar. Yazarlardan, yazının detaylarını ve tarihini bildirecek şekilde imzalı bir beyan ile başvurmaları istenir. Çalışmalar deney hayvanı kullanımını içeriyorsa, hayvan bakımı ve kullanımında yapılan işlemler yazı içinde kısaca tanımlanmalıdır. Deney hayvanlarında özel derişimlerde ilaç kullanıldıysa, yazar bu derişimin kullanılma mantığını belirtmelidir. İnsanlar üzerinde yapılan deneysel çalışmaların sonuçlarını bildiren yazılarda, Kurumsal Etik Kurul onayı alındığını ve bu çalışmanın yapıldığı gönüllü ya da hastalara uygulanacak prosedürlerin özelliği tümüyle kendilerine anlatıldıktan sonra, onaylarının alındığını gösterir cümleler yer almalıdır. Yazarlar, bu tür bir çalışma söz konusu olduğunda, uluslararası alanda kabul edilen kılavuzlara ve TC. Sağlık Bakanlığı tarafından getirilen ve 28 Aralık 2008 tarih ve 27089 sayılı Resmi Gazete'de yayınlanan "Klinik araştırmaları Hakkında Yönetmelik" ve daha sonra yayınlanan 11 Mart 2010 tarihli resmi gazete ve 25518 sayılı “Klinik Araştırmalar Hakkında Yönetmelikte Değişiklik Yapıldığına Dair Yönetmelik” hükümlerine uyulduğunu belirtmeli ve kurumdan aldıkları Etik Komitesi onayını göndermelidir. Hayvanlar üzerinde yapılan çalışmalar için de gereken izin alınmalı; yazıda deneklere ağrı, acı ve rahatsızlık verilmemesi için neler yapıldığı açık bir şekilde belirtilmelidir. Hasta kimliğini tanıtacak fotoğraf kullanıldığında, hastanın yazılı onayı gönderilmelidir. Material and Methods: Wistar albino rats were exposed to WAS (WAS group). After exposing the animals to WAS (WAS plus AGE group), 250mg/kg AGE was injected i.p.. Dermal mast cells were stained with toluidine blue and investigated using light microscopy. Results: The number of both granulated and degranulated mast cells was higher in theWAS group, when compared to the control group. The number of mature granulated and degranulated mast cells was lower in the WAS plus AGE group when compared to the WAS group. Conclusion: Chronic AGE treatment reduced WAS-induced infiltration and activation of mast cells in the dermis and may provide a useful therapeutic option in stress-induced skin disorders. Keywords: Dermis, Mast cell, Water avoidence stress, Aqueaus garlic extract SULU SARIMSAK ÖZÜTÜNÜN SUDAN KAÇINMA STRESİNİN TETİKLEDİĞİ DERİDEKİ MAST HÜCRE DEGRANÜLASYONUNA KARŞI KORUYUCU ETKİSİ ÖZET Amaç: Bu çalışmanın amacı sulu sarımsak özütü (=aqueaous garlic extract, AGE)’ nün, sudan kaçınma stresi (SKS)’ nin tetiklediği derideki mast hücre degranülasyonu üzerindeki etkilerinin araştırılmasıdır. Gereç ve Yöntem: Wistar albino sıçanlar, kronik sudan kaçınma stresi (SKS grubu)’ ne maruz bırakıldı. Hayvanlar SKS’ne maruz bırakıldıktan sonra 250mg/kg AGE i.p. olarak enjekte edildi (SKS + AGE grubu). Dermal mast hücreleri toluidin mavisi ile boyanarak ışık mikroskopi düzeyinde incelendi. Sonuçlar: SKS grubunda hem granüllü hem de degranüle olmuş mast hücrelerinin sayısı kontrol grubundakilere oranla artmıştır. SKS + AGE grubundaki granüllü ve degranüle olmuş olgun mast hücrelerinin sayısı SKS grubundakilere oranla azalmıştır. Tartışma: Kronik AGE tedavisi SKS’nin tetiklediği dermisteki mast hücre artışını ve yoğunluğunu azaltmıştır ve stresin tetiklediği deri rahatsızlıklarında tedavi edici ajan olarak kullanımı çalışmalara açık görülmektedir. The role of mast cells in inflammatory and allergic reactions including stress-induced inflammatory diseases is well documented. Mast cells are implicated in skin disorders such as atopic dermatitis, psoriasis and these diseases are either induced or worsened by stress6. The skin of patients with atopic dermatitis contains significantly higher numbers of mast cells than intact skin7. In psoriasis, the number of mast cells is significantly elevated in both lesioned skin and intact skin5. Mast cells release inflammatory mediators upon activation, such as histamine, proteases, prostaglandins, leukotrienes and cytokines that have potent vasodilatory, and inflammatory properties8. For example, histamine from mast cells increases vascular permeability upon activation8 and stimulates cutaneous sensory nerves9. Stimulation of unmyelinated sensory nerves leads to the release of neuropeptides such as substance P10 which act on mast cells, blood vessels11 and leukocyte infiltration12. It has been recently suggested that the skin may have its own equivalent of the hypothalamic–pituitary– adrenal (HPA) axis13 because the corticotropine releasing hormone (CRH) and its receptors were shown to be present in the skin8. METHODS Animals: Adult female Wistar albino rats weighing 200-250 g were housed individually in a light- and temperature-controlled room on a 12/12 h light/dark cycle, and fed a standard pellet lab chow. Stress Procedure: The rats were handled daily by the same investigator for 2 weeks before the start of the study and were then submitted to WAS as described previously24. The procedure was as follows: rats were placed on a glass platform (8X6 cm) in the middle of a plastic container with a diameter of 90 cm and height of 50 cm filled with water of 25ºC to 1 cm below the level of the platform. The rats avoided the aversive stimulus (water) by staying on the platform for 2 h. All experimental procedures were performed between 8:00 and 10:00 to minimize the effect of circadian rhythms. Preparation of aqueous garlic extract: Peeled garlic (30 g) was crushed with distilled water in a mortar. The crushed material was carefully decanted by pressing and 60 ml of aqueous extract was extracted. One mililiter of aqueous extract contained 500 mg of garlic materials. Experimental groups: Three groups of rats were treated as follows: 1) Control group (n=8): rats were placed on the platform in a container without water for 2 h and were injected with a saline solution 2) Stress (WAS) group (n=8): rats were exposed to WAS for 2 h daily for 5 days after receiving an injection of a saline solution each day. 3) Stress exposed, AGE treated group (WAS+ AGE group) (n=8): After exposing them to WAS, 250mg/kg AGE was injected ip. for 3 days into the animals. During the experimental procedures the animals were fed with a standard pellet lab chow and water ad libitum. At the end of the experiments, the rats were euthanized by decapitation. Many beneficial, health-related biological properties are attributed to garlic, among them, that garlic preparations including aged garlic, prevent tumour promotion14-16, cardiovascular diseases17, liver damage 18 and aging19 which are considered to be associated with oxygen radical and lipid peroxidation. The intrinsic antioxidant activity of garlic20, garlic extracts21 and some garlic constituents22 have been widely documented in vivo23 and in vitro22. Considering the fact that psychological stress induces or worsens various skin conditions, we investigated whether water avoidance stress (WAS) affects the occurrence of mast cells in the skin and their degranulation and if so, whether aqueous garlic extract can reduce mast cell activation. Histological preparations: Pieces of skin from the back of approximately 2 cm2 (skin were taken from the same location in all rats) including the hypodermis and were fixed in 10% formaldehyde and then routinely processed for paraffin embedding. The sections were examined at 400X magnification with a BX50 photomicroscope (Olympus, Tokyo, Japan). were increased in the WAS group, and numbers of activated mast cells were increased in the WAS group when compared with the control groups. Numbers of both granulated and degranulated mast cells were significantly reduced in the WAS + AGE group when compared with the WAS group (Fig. 2). Mast cell counting and statistical analysis: Sections of the skin were evaluated for mature granulated mast cells and degranulated mast cells. Granulation was considered according to the fullness of the mast cells with granules whereas degranulation meant when the granules were out of the cell and observed nearby the cells. Ten randomly taken sections (the first, eleventh, twenty-first, etc) from each animal were selected and the mast cells were counted without knowing to which treatment group the rat belonged. In each section, mast cells containing metachromatic granules were counted separately at 400x magnification in five consecutive dermal areas by one or more observer and the observers were blinded to the treatment of the animals. An eye-piece graticule (size 0.0785 mm2) was used in order to avoid overlap of areas to be counted. It had a vertical and a horizontal axis, so that the area was divided into 4 parts, which facilitated counting. Areas selected in each region were surveyed for mast cells and the mast cell density was then expressed as cell number per unit area. The data were analyzed by using one-way analysis of variance (ANOVA). Differences between groups were determined with the Dunnett’s multiple comparisons test and the data were expressed as mean ± standard error of the mean. The significance of differences was taken as the level p < 0.05. RESULTS In the control (Fig.1a) mast cells stained with toluidine blue were mostly intact. In the WAS group (Fig 1b) both the granulated and degranulated mast cells were increased in number. In the WAS + AGE group (Fig. 1c) most mast cells were granulated with a darkly stained cytoplasm and a few of them were degranulated. Quantitative analysis of the numbers of mast cells that were granulated and degranulated in the different treatment groups is shown in Fig. 2. DISCUSSION The results of the present study demonstrate that AGE has a protective effect on chronic psychological, nontraumatic stress-induced numbers of mature mast cells in dermis of rats. Tanaka et al33 reported that Allium vegetables, including garlic, inhibit the release of ß-hexosaminidase, which is correlated with histamine release, in rat basophilic leukemia cells (RBL-2H3), suggesting an anti-allergic effect34 . The antiallergic activity of (Z)-ajoene might be due to its inhibition of chemical mediator release. It was also found that AGE has an anti-allergic property35. Histamine release in the rat basophil cell line RBL-2H3 was induced by mouse anti-trinitrophenyl (TNP) monoclonal antibody and the TNP-bovine serum albumin (BSA) hapten carrier complex. AGE added to the culture medium at doses of 1.25, 2.5 and 5.0 g/100 g significantly inhibited the antigen specific histamine release by 50, 80 and 90%, respectively. Oral administration of AGE (10 mL/kg) also decreased 25–45% of the ear swelling, used as an index of immunoglobulin (Ig) E–mediated skin reaction, caused by intravenous administration of anti-TNP IgE antibody and subsequent picryl chloride painting on the ear in mice27. In the present study, WAS induced an increase in number and degranulation of mast cells in the rat Psychological and physical stress has been shown to suppress immune functions. Yokoyama et al25 reported that an AGE preparation containing vitamins B-1 and B-12 restored physical stress–induced immune suppression in mice. Recently, it was determined that the effect of AGE on immune suppression caused by psychological stress26,27 is determined. Activation of mast cells has been shown in different stress models. Repetitive exposure to odors28, Pavlovian conditioning29 and isolation stress conditions30 cause secretion of histamine from mucosal mast cells. We have previously observed that stress-induced inflammatory changes such as mast cell degranulation are mediated by capsaicinsensitive sensory neurons in the dermis31. These results suggest that WASinduced degranulation of mast cells occurs via activation of inflammatory reaction and, stimulation of mast cell to give anti allergic compounds such as histamine to the extracellular matrix and these can be prevented by treatment with exogenous AGE via modulation of immune, antiinflammatory and antiallergic reactions. 12. 13. 14. In conclusion, systemic treatment of AGE appears to reduce the WAS-induced increase in numbers and degranulation of mast cells in the skin. Further studies on this subject must be carried out in order to establish the effects of AGE as a possible therapeutic agent in many skin diseases such as psoriasis and atopic dermatitis. 15. 16. 17. Yöntem ve Bulgular: Sözü edilen yıllar arasında küretaj ve histerektomi raporlarına ulaşılan 48 hastanın patoloji raporları derecelendirme açısından incelendi. Uyum; derece 1 için %70, derece 2 için %70, derece 3 için %100 ve bütün olgular için %77 olarak bulundu.Derece 3 tümörlerde uyum, derece 1 ve 2 ye göre belirgin olarak yüksekti. Sonuç: Bazen endometrial adenokarsinomların diferansiasyon dereceleri küretaj materyali sonrası histerektomi materyalinde değişiklik gösterebilmektedir. Bu durum küretaj materyalinde özellikle derece 1 ve 2 tanısı alan hastaların yönetiminde karışıklık yaratabilir. Methods and Results: Curetting materials and subsequent hysterectomy specimens from the pathology reports of 48 patients were studied.Concordance was 70% for grade 1, 70% for grade 2, 100% for grade 3 carcinomas, and the overall concordance 77%.The concordance for grade 3 tumours was significantly higher than for grade 1 and 2. Conclusion: The grade of endometrial adenocarcinoma sometimes differs in the initial curettage sample from that in the final hysterectomy specimen.This may have important implications for patient management especially for tumours that yield grade 1 and 2 biopsies. Endometrial adenokarsinomların küretaj ve histerektomi spesmenlerinde fıgo tümör diferansiasyon derecesi uyumu göstermesine rağmen istikrarlı bir şekilde geçerliliğini sürdürmektedir4,5. nonskuamöz, nonmoruler solid büyüme patterni gösteren tümörler derece 1, %6-%50 solid büyüme patterni gösteren tümörler derece 2 ve % 50 den fazla solid büyüme patterni gösteren tümörler de derece 3 olarak kabul edildi.Yapısal derece ile uyumsuz olan nükleer derece, 1 veya 2 olan tümör derecesini bir derece arttırdı..Seröz ve berrak hücreli adenokarsinomlarda nükleer derecelendirme ön planda tutuldu. Skuamöz diferansiasyon gösteren tümörlerde, glandüler komponentin nükleer derecesine göre derecelendirme yapıldı. Endometrial kanserlerde ilk histolojik tanı; küretaj veya biyopsi ile elde edilen endometrial örneklere koyulmaktadır. Endometrial adenokarsinomu olan hastaların uygun tedaviyi alabilmeleri için biyopsi materyalinde belirlenen derecenin bütün tümörü doğrulukla yansıtması önemlidir6. Bu konu özellikle ‘derece 3’ tümörlerde daha önemli bir hal almaktadır zira bu hastalara genellikle daha radikal cerrahi evreleme ve radyoterapi verilmesi gerekmektedir6. Tümörün derecesine ek olarak raporlardan hastanın yaşı ve materyal (küretaj), konsültasyon tipi de elverdiği ölçüde kayıt edildi. Bu çalışmada; 1999 ile 2005 yılları arasında tanı almış 48 olguda, küretaj örneğindeki FIGO derecesinin daha sonra elde edilen histerektomi spesmenindeki bütün tümörün derecelendirmesi ile karşılaştırılarak,küretaj materyalindeki derecelendirmenin öngörücü değeri araştırılmaktadır. BULGULAR Elemelerden sonra geriye kalan 48 hastanın biyopsi sırasındaki yaş ortalaması 56(34-77) arası olarak tespit edilmiştir. GEREÇ VE YÖNTEM Zeynep Kamil Hastanesi Patoloji Bölümünde kayıtlı toplam 177 endometrial adenokarsinom arasından hem endometrial hem de histerektomi materyali olan 48 hastaya ait raporlar seçildi. Histerektomi spesmeni olmayan biyopsi raporları , biyopsi raporları olmayan histerektomi raporları çalışma dışı bırakıldı. Endometrial örnekleme ve histerektomi raporu bulunan toplam 52 adet olgunun ikisi maliğn mikst müllerian tümör tanısı aldığı ,birinde de rezidü tümör kalmadığı, bir diğeri de ilk spesmeninde yanlış tanı aldığı için elimine edildi. Konsülte edilen 5 olgudan 2 sinin (%40) histerektomi spesmeninde FIGO derecesi değişmiş, 3 ünde (%60) aynı kalmıştır (Tablo I). Toplam 48 hastanın küretaj materyalleri; 27(%56) si derece 1, 10 (%21)u derece 2 ve 11(%23)i derece 3 olarak tanımlanmıştır. Derece 3 olarak tanımlanmış 11 olgunun 2 sinde tanı berrak hücreli karsinom, 1 inde de seröz karsinomdur. İlk materyallerine derece 1 olarak tanı konmuş 27 adet olgunun histerektomi materyallerinden 7(%26)sinde derece 2,1(%4)inde de derece 3 olarak değişiklik olmuştur.(Tablo II). İlk örnekleme materyali derece 2 olan 10 olgunun histerektomi materyalinden 3(%30)ünde derece 3 olarak değişiklik görülmüştür (Tablo III). Küretajları derece 3 olarak belirlenmiş 11 adet olgunun histerektomi materyallerinde derecelendirme değişikliği olmamıştır. Kırksekiz hastadan 43 hastanın materyali direk olarak laboratuarımızda incelenirken, 5 hastaya ait materyal (hazır parafin blok ve/veya hazır H+E boyalı lam halinde), konsülte edilmek üzere bölümümüze ulaştırılan olgular arasından seçildi. Seçilmiş 48 olguya ait biyopsi ve histerektomi raporlarından FIGO dereceleri kaydedildi. Derecelendirilmemiş olgulara ait histolojik kesitler patologlardan biri (HÇ) tarafından yeniden değerlendirilerek derece verildi. Bütün spesmenlerin derecelendirilmesinde; yapısal ve nükleer özelliklerin değerlendirildiği FIGO sistemi kullanıldı7.Bu sistem kullanılarak, %5 ve daha az oranda Bu sonuçlara göre derece 1, 2 ve 3 tümörlerde küretaj materyalinin pozitif öngörücü değeri sırasıyla; %70, %70 ve %100 olarak saptanmıştır.Derece ayrımı yapılmaksızın bütün olgularda pozitif öngörücü değer %77 olarak hesaplanmıştır. 110 Marmara Medical Journal 2005;18(3);109-112 Handan Çetiner ve ark. TARTIŞMA Endometrial karsinomlu olgularda; tümör diferansiasyonunun prognostik önemi 1-3 yıllardan beri bilinmektedir . Ancak bazı çalışmalarda yaş, klinik evre ve myometrial invazyon derinliği gibi diğer prognostik faktörlerle yüksek oranda korelasyon gösterdiği ve bu faktörler eşitlenerek düzeltildiğinde histolojik derecenin sağ kalım veya rekürrens süresini gösterme gücünün azaldığı ifade edilmektedir8,9. Bununla birlikte Greven ve arkadaşlarının yaptığı bir çalışmada metastazı olan (evre 3) hastalarda bile histolojik derecenin prognozu bağımsız olarak belirlediğine dair sonuçlar elde edilmiştir10. Derecelendirme yapılırken intrave interobserver uyumsuzluklar;özellikle nükleer derecelendirme için sözkonusu olmasının yanısıra5 tümör içerisinde diferansiasyon heterojenitesi de mevcut olabilir11. Bütün bu olası nedenlerle küretaj ve histerektomi spesmenlerinin derecelendirilmeleri arasında uyumsuzluk görülebilir. Çalışmamız kapsamındaki 5 adet konsültasyon olgusundaki uyum makroskopisi bölümümüzde alınmış materyalle orantılı olarak %71 dir. Ancak olgu sayısının sadece 5 olması bu uyumun yanıltıcı olabileceğini düşündürtmekte, patoloğun alışık olmadığı bir takip ve kesit kalitesinin hata oranının konsültasyon olgularında artırabileceği akla yakın gelmektedir. Histolojik derecenin; myometrial invazyon, klinik evre gibi parametrelerle korele olduğunu belirleyen çalışmalar göz önünde bulundurulduğunda 8, 9 küretajda belirlenen tümör derecesinin prognostik parametre olmanın ötesinde operasyonun şeklinin de belirleyicisi olma yönünde önemi vardır.Bu nedenle küretaj materyalinin derecelendirilmesinin, histerektomi Petersen ve arkadaşlarının12 yaptığı çalışmada biyopsi derecesinin pozitif öngörücü değeri derece 1, 2 ve 3 için sırasıyla %60, %70 ve %84 olarak tespit edilmiştir. Oakly ve arkadaşlarının13 yaptığı başka bir çalışmada öngörücü değerler yine sırasıyla %88, %10 ve 111 Marmara Medical Journal 2005;18(3);109-112 Handan Çetiner ve ark. Endometrial adenokarsinomların küretaj ve histerektomi spesmenlerinde fıgo tümör diferansiasyon derecesi uyumu %67 olarak rapor edilmiştir.Mitchard ve arkadaşlarının 6 yaptığı çalışmada da pozitif öngörücü değerler yine sırasıyla %45, %63 ve %75 tir. 4. Bizim çalışmamızda da Petersen ve Mitchard ve arkadaşlarının sonuçları ile uyumlu olarak; tümör derecesi yükseldikçe öngörücü değer yükselmektedir.Derece 3 tümörlerde %100e ulaşan pozitif öngörücü değerin nedeni bu grupta olgu sayısının az olması olabileceği gibi, bu grubun endometrioid karsinomun dışında seröz ve berrak hücreli karsinomları içermesi de olabilir.Derece 1 ve 2 tümörlerde yapısal heterojenite nedeniyle nispeten kısıtlı miktardaki küretaj materyali yanıltıcı olabiliyorken, derece 3 sayılan seröz ve berrak hücreli karsinoma ait materyal az bile olsa, belirgin nükleer yapıları nedeniyle tümörün genelinin derecesini temsil etmesi yönünden daha elverişli sayılabilir. 5. 6. 7. 8. 9. Çalışmamızda elde ettiğimiz; derece ayrımı yapılmaksızın ; bütün küretaj ve histerektomi materyalleri arasındaki uyuma ait %77 lik pozitif öngörücü değer literatürdeki bir çok çalışma ile uyum göstermektedir14-16. 10. 11. Bu çalışmada elde edilen sonuçlar ve literatür bilgileri ışığında derece 1 ve 2 olarak belirlenmiş tümörlerde operasyonun;tümörün genelinin daha az diferansiye olabileceği göz önünde bulundurularak planlanması ve bu plan içeriğinde mutlaka intraoperatif patoloji konsültasyonunun yeralmasının hastanın yönetiminin optimum olması yönünde fayda sağlayacağı söylenebilir. 12. 13. 14. Gereç ve Yöntem: 1 Ocak – 31 Aralık 2004 tarihleri arasında acil servisimize başvuran travma vakaları retrospektif olarak protokol defteri ve hasta dosyalarından incelendi. Toplamda çalışmaya alınma kriterlerine uygun 636 vaka kaydına ulaşıldı. Çalışmaya alınan vakaların yaş, cinsiyet, travma özellikleri, yatış, taburculuk, mortalite ve morbidite oranları göz önüne alındı. Vakalar lokal ve multipl travma olarak iki ana gruba ayrıldı. Bu parametreye göre yatış oranları ve bu oranları belirleyen koşullar hesaplandı. Analiz için chi-kare ve Fischer’s Exact test kullanıldı, p<0.05 olan istatistiksel farklılıklar anlamlı olarak kabul edildi. Bulgular: Acil servise başvuran 636 travma hastası bu çalışmaya alındı. Vakaların çoğu erkek (%67) olup medyan yaşları 42’dir. Başvuran lokal ve multiple yaralanma vakalarının sayısı arasında anlamlı fark bulunmazken, multipl travma hastaları anlamlı olarak daha fazla yatırılmıştır. En sık yaralanma mekanizması düşmelerdir. Yandaş hastalık varlığı belirgin olarak fazladır. En fazla travma hastası Nisan- Haziran 2004 arasında başvurmuştur. Sonuç: Vaka dağılımları ve demografik özellikleri daha önce başka Acil Tıp Anabilim Dalları tarafından yapılan çalışmalardaki sonuçlarla benzerlik göstermekle beraber aynı bölgede bulunan diğer hastanelerdeki çalışmalara göre hastanemiz acil servisine daha yaşlı, yandaş hastalığı daha fazla olan düşme hastaları başvurmaktadır. Anahtar Kelimeler: Travma, Demografi, Hasta profili DEMOGRAPHIC CHARACTERSITICS OF TRAUMA PATIENTS OF THE EMERGENCY DEPARTMENT OF MARMARA UNIVERSITY HOSPITAL ABSTRACT Objective: This study aims to describe the characteristics of trauma patients admitted to the Marmara University Hospital , Department of Emergency Medicine. Materials and Methods: Patients admitted to our ED between 1st January – 31st December 2004 were retrospectively analysed from the admission charts and patient files. 636 cases met the inclusion criteria. Age, sex, trauma properties, admission, disposition, mortality and morbidity frequencies have been calculated. Cases have been divided into local and multiple trauma groups. Also the admission frequencies and the factors affecting decision for admission have been searched.Chi-square and Fischer’s Exact tests were used in statistical analysis, p<0,05 accepted to be statistically significant. Results: Six hundred and thirty six trauma patients have met the criterion to be included in the analysis. Most of the cases were men (67%) and the median age was 42. Statistically there was no difference between the number of the local and multiple trauma cases. Hospitalization rates of multitrauma patients was significantly higher. The most frequent mechanism of trauma was falls. Rates of associated diseases were significantly higher in our patients. Most patients were admitted between April and June 2004. Conclusion: Dispersion of the cases and demographics of the patients seemed to resemble the conclusions that have been reached by the other emergency medicine departments of the universities in other cities, but our patients were older and had more comorbid diseases when compared with the state hospitals in our city. Leading cause of trauma was falls in our clinic in 2004. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri belirtilen, ancak kimlik bilgisi ya da dosya numarası olmayan, dosyası kayıp ya da hiç dosya açılmamış protokol defter kayıtları da çalışmaya alınmadı. Bu şekilde hiçbir bilgiye ulaşılamayan protokol defteri hastalarının sayısı 21 idi. Çalışmaya alınan vakaların yaş, cinsiyet, travmanın oluştuğu vücut bölgeleri ve kapsadığı bölgelerin sayısı, lokal ve multitravma görülme sıklığı, yandaş hastalıkların varlığı, yatış ve taburculuk oranları, morbidite ve mortalite durumu gibi bazı demografik ve epidemiyolojik özellikleri kaydedildi. Yatış oranları ve endikasyonlarının, bilim dalları arasındaki farklılıkları ve travma vakalarının izleminde acil servisin rolünü belirleyebilmek amacıyla da vakalar lokal ve multipl travma olarak iki ana gruba ayrıldı. Multipl travma hastası, “Injury Severity Score (ISS)”5 içerisinde tanımlandığı üzere ISS skorunun 16 ya da daha fazla olmasıdır6. Amacımız, hastaların yaralanmalarının ağırlıklarını değerlendirerek, buna uygun şekilde hastaneye yatırılıp yatırılmadıklarını gözlemlemek olduğundan, elimizdeki verilerle uygulayabildiğimiz “Abbreviated Injury Scale (AIS)” adı verilen ve “Otomobil Güvenliğinin Medikal Yönleri Komitesi”nce ilk olarak 1971 yılında tanımlanmış bir başka skorlama sisteminde belirtilmiş multipl ve lokal travma hastası önermelerini kullandık. Bu skorlama sisteminde vücut, baş-yüz-boyun, toraks, abdomen ve ekstremite olmak üzere 4 ana vücut bölgesine ayrılmakta ve en az ikisinde travma varlığı multipl travma olarak tanımlanmaktadır7. Bu vücut bölgelerindeki yaralanmalar, kendi bölgelerine sınırlı kaldıkları sürece lokal yaralanma olarak sınıflandırılmıştır. Bu skorlama sistemlerinden yola çıkarak, iki ve ikiden fazla vücut bölgesinden aynı anda yaralanması olan hastalar, multipl travma hastası olarak kabul edildi. Elde edilen verilerin istatistiksel analizi için Ki-kare ve Fischer’s Exact test kullanıldı, p<0.05 anlamlı fark olarak kabul edildi. Bu analiz esnasında, hem aylara göre travma oluş mekanizmalarının karşılaştırılması için düşmeler dışındaki vaka sayılarının; hem de lokal ile multipl travma hastalarının hastanemiz acil servisindeki mortalitelerini GİRİŞ Travma, gençlerin hastalığı olup 1–44 yaş arasında gerçekleşen ölümlerin sebepleri arasında ilk sıradadır1. Travma ve yaralanmaya bağlı hasar, Acil Servise en sık başvuru nedenlerinden biridir. 1995 yılında ABD’de hastaların %37’si yaralanmaya bağlı hasar nedeniyle Acil Servise başvururken, tüm ölümlerin %6’sı ve hastane taburculuklarının %8’i bu sebeple gelen hastaları kapsamaktadır2. Ülkemizde bu genişlikte bir çalışma olmamakla beraber Acil Tıp Anabilim Dalı bulunan diğer üniversitelerden yapılmış yayınlarda tüm hastaların %7-20’sinin travma hastası olduğu belirlenmiştir3,4. Bu çalışmayla, Marmara Üniversitesi Acil Tıp Anabilim Dalı Acil Servisi’ne başvuran travma hastalarının demografik ve epidemiyolojik özelliklerini retrospektif olarak saptayarak ülkemizin bu konudaki verilerine katkıda bulunmak, hastanemiz ve acil servisimizin ihtiyaç ile eksikliklerini belirlemek ve buna uygun düzenlemeleri yapmak amaçlanmıştır. Ayrıca hastaların travmalarının ağırlığı ile hastane içi takipleri arasında acil servis ya da diğer bölümler arasında belirgin bir fark olup olmadığı tespit edilmeye çalışılmıştır. GEREÇ VE YÖNTEM 1 Ocak – 31 Aralık 2004 tarihleri arasında acil servisimize başvuran travma vakaları retrospektif olarak protokol defteri ve hasta dosyalarından incelendi. Travma hastalarının belirlenmesi için, hastanemiz Acil Servis Hasta Kabul ve Muayene Kartları’nda yer alan, adli vaka ve travma vakalarında işaretlenmesi zorunlu “Travma” bilgi kutuları kullanıldı. Bu bilgi kutusu işaretli tüm hastalar çalışmaya dâhil edildi. Bunun yanı sıra, “Travma” kutusu işaretli olmayan ancak hastayı görmüş olan doktorun dosyaya, protokol defterine ya da tanı bölmesine “travma vakası” olduğunu belirttiği tüm hastalar da çalışmaya alınarak toplamda 636 vaka kaydına ulaşıldı. Retrospektif tarama sırasında, sadece “travma vakası” olması dışında ek bilgi ya da açıklamaya ulaşılamayan vakalar listeleme dışında bırakıldı. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri karşılaştırabilmek için hastanemizde mortalite gelişen hasta sayısının istatistikî değerlendirme için yeterli örneklem boyutu oluşturamadığı belirlenmiştir. BULGULAR 1. Başvuru sayısı 2004 yılı içerisinde, Marmara Üniversitesi Acil Tıp Anabilim Dalı Acil Servisine toplam 23.648 hasta başvurmuştur. Başvuran hastaların 636’sı (%3) travma hastasıdır. 2. Cinsiyet ve Yaş Travma hastalarının %33’ü (210) kadın, %67’si (426) erkektir (Şekil 1). Travma hastalarının medyan yaşı erkeklerde 42, kadınlarda ise 48’dir. Aylara göre travma oluş mekanizmaları Travma oluş mekanizmaları açısından yapılan değerlendirmede, en çok travma hasta başvurusu yapılan Ekim ve Mayıs aylarında ilk sırayı düşme-çarpma-burkulmalar gibi travma etkenleri almakta (sırasıyla %70 ve %69), ardından delici kesici alet yaralanmaları (sırasıyla %29 ve %14), trafik kazaları (sırasıyla %7 ve %13) ve diğer sebepler gelmektedir. Vaka sayısının en fazla olduğu ve diğer aylardan anlamlı olarak fazla olan Mart, Mayıs ve Ekim aylarında düşme vakalarının sayısı diğer vakalara oranla anlamlı olarak daha fazladır (p<0.025). Mart ayında gelen delici kesici alet yaralanmaları sayısı ise diğer aylara oranla anlamlı olarak daha düşüktür (p<0.025) (Şekil 3). Aylara göre travma vaka sayıları Aylara göre travma vaka sayıları karşılaştırıldığında (Şekil 2), ilkbahar ve sonbahar aylarında vaka sayıları anlamlı olarak artarken (Mart 60, Nisan 49, Mayıs 94, Haziran 62, Ekim 92, Kasım 49), yaz aylarında (Temmuz 31, Ağustos 14, Eylül 25)vaka sayısının belirgin derecede azalmakta olduğu belirlenmiştir. Yılın TemmuzAğustos-Eylül aylarını içeren 3. çeyreğinde, yıllık travma vakalarının sadece %11’i başvurmuşken; Nisan-Mayıs-Haziran döneminde %32’si, Ocak-Şubat-Mart döneminde %25’i, Ekim-Kasım-Aralık döneminde ise %31’i tedavi almıştır. Temmuz-Ağustos-Eylül dilimi, anlamlı olarak en az travma vakası gelen zaman aralığıdır (p<0.001). En çok travma vakası ise Mayıs ve Ekim aylarında başvurmuştur (Şekil 2). Travmanın gerçekleştiği vücut bölgesi sayısı Yaralanmanın gerçekleştiği vücut bölgelerinin sayısı kullanılarak hastalar lokal ve multipl travma hastası olmak üzere 2 ana gruba ayrılmıştır. Bu sınıflamaya göre 636 hastanın 346’sı (%54) lokal, 290’ı (%46) ise multipl travma olarak belirlenmiştir (Şekil 4). ilgili birimlere yatırılmıştır. İzole batın travmalı 12 hastanın hepsi acil serviste yatırılarak izlenmiştir (Şekil 6). Lokal travmaların %72’si (249) ekstremitelerden, %21’i (72) baş-boyun bölgesinden, %3,7’si (13) torakstan, %3,4’ü (12) abdomenden gerçekleşmiştir (Şekil 5). Vakaların 220’si (%75) 2 bölgeyi, 65’i (%22) 3 bölgeyi, 5’i (%0.01) ise tüm vücut bölgelerini içermekte olan multipl travma hastalarıdır. Multipl travma vakalarında en sık görülen travma vücut bölgesi kombinasyonu 3 ve daha fazla vücut bölgesini içeren multitravma tipidir(138 vaka %47,5)(Şekil 7). Lokal ekstremite travmalarının %6’sı (15), baş-boyun travmalarının %10’u (7), toraks travmalarının %7’sinde (1) yatış endikasyonu konulmuş ve 290 vakanın 26’sı (%8,9) dış servislere, 84’ü (%28,9) ise acil servise yatırılmış olup toplamda hastaların %37,8’ine (110) yatış endikasyonu konulmuştur. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri hastalarını hastane katlarındaki servislerden ziyade acil serviste yatırma ve izlemeye yönelik bir eğilim belirlenmiştir (p<0.05). 26 dış servis yatışı vakasının 15’i (%57,7) Genel Cerrahiye, 8’i (%30,7) Nöroşirurjiye, 3’ü (%11,6) ortopediye yapılmıştır. Acil serviste yatırılan 84 hastanın 12’si (%15) aynı gün opere edilmiştir. Dış servislere yatırılan 26 multipl travma hastasından sadece ikisi aynı gün içerisinde opere edilmiştir. 6. Travma oluş mekanizmaları Travma oluş mekanizmalarına göre sınıflandırma yapıldığında, düşmeler (%70) en sık sebeptir. Bunu sırasıyla burkulmaezilme, darp, delici-kesici aletle yaralanma, motorlu taşıt yaralanması takip etmektedir. Bu vakaların biri lokal diğer 4’ü multipl travma hastasıdır. 7. Yandaş hastalıklar Hastaların operasyon ve operasyon sonrası mortalite düzeyini arttıran, yoğun bakım ünitesinde entübe olduktan sonra ventilatörden ayrılma (weaning) süresini uzatan hastalıklar yandaş hastalık olarak kabul edilmiştir. Hastaların dosyaları taranarak bu kategorilerde riskleri arttırdığı bilinen ek hastalıkları not edilmiştir. Yandaş hastalık varlığına göre hastaların sınıflandırılması için ilgili dosya bilgilerinin retrospektif taraması sırasında lokal travma hastalarının %57’sinin (197), multipl travma hastalarının ise %51’inin (148) ya dosyalarına ya da dosyalarındaki ilgili bilgilere ulaşılamamıştır. Eğer hastanın özgeçmişi ile ilgili bir özellik olup olmadığı belirtilmeyip hasta kartında bu bölge boş bırakılmışsa, “bilgi yok” olarak kabul edilmiş ve hesaplamadan çıkarılmıştır. Ancak geri kalan hasta grubunda, lokal travma vakalarının %26’sında, multipl travma vakalarının ise %42’sinde diabetes mellitus, konjestif kalp yetmezliği, koroner arter hastalığı, hipertansiyon, geçirilmiş serebrovasküler olay, malignite, epilepsi, dejeneratif beyin hastalığı ve senilite gibi en az 1 yandaş hastalık mevcuttur (Şekil 8). 346 lokal travma vakasının 4’ünde bilinç bulanıklığı ya da koma hali mevcut olup, bu 4 vaka da izole kafa travmasıdır ve 1 tanesi acil serviste eksitus olmuştur (%0,2). 290 multipl travma vakasının 4’ü (%1) acil servisten eksitus raporu almıştır. Bunların 2’si yüksekten düşme, 2’si ise motorlu taşıt kazasıdır. Motorlu taşıt kazası vakalarından biri eks-duhul olup acil servis dışında eks olmuş ve sadece ölüm raporunu acil servisten almıştır. TARTIŞMA Hastanemiz acil servisine başvuran 636 travma hastası, toplam başvuran hasta popülâsyonu içerisinde %3’lük bir dilime karşılık gelmekte olup, geniş prospektif çalışmalarla belirlenmiş Türkiye dışı yayınlardaki değerlerden oldukça düşüktür2. Bunun sebebi Marmara Üniversitesi Tıp Fakültesi Hastanesi’nin bulunduğu yer ve hizmet verdiği bölgede yer alan, travma merkezi olarak inşa edilmiş Sağlık Bakanlığı Araştırma ve Eğitim Hastaneleri’nin (SBAEH) varlığıyla açıklanabilir. Mevcut konumu itibariyle, özellikle motorlu taşıt kazası vakalarının ilk transport edilebileceği konumdan uzakta bulunan hastanemiz, diğer primer bakım veren hastanelerden sevk zinciri 8. Eksitus oranları Tüm travma hastaları içerisinde, toplam 5 vakaya acil servisten ölüm raporu verilmiş, bunların 4’ü (1 eks-duhul hariç) acil serviste eksitus (%0,6) olmuştur. Eksituslardan bir 117 Marmara Medical Journal 2005;18(3);113-122 Haldun Akoğlu ve ark. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri yoluyla, üniversite hastanesinde bakımı uygun görülmüş yandaş hastalıkları fazla olan hastaları almaktadır. Yandaş hastalıkları ya da travmasının ağırlığı açısından SBEAH’lerinde tedavisi uygun görülen ve bu kurumlarda tamamlanan hastaların sayısının çokluğu nedeniyle hastanemize başvuran travma hasta sayısı da düşük kalmaktadır. Burada, teknik yetersizliklerin engellemesi ve hastane içi protokollerce belirlenmiş bir travma timi, travma ameliyathanesi ve travma yaklaşımının olmayışının çok büyük etkisi olduğunu düşünmekteyiz. bölündüğünde örneklem boyutlarının çok küçülmesi olabilir. Lokal ve multiple travma hastalarının sayısı birbirine yakındır. Ancak yöntem bölümünde de belirttiğimiz gibi multipl travma tanımlaması aşamasında birtakım zorluklarla karşılaşılmıştır. Multipl travma hastası, “Injury Severity Score (ISS)”5 içerisinde ISS skorunun 16 ya da daha fazla olması6 şeklinde tanımlanmış olsa da bu tanım, hastaların sınıflandırılmasından ziyade, uzun dönem mortalite ve morbiditelerinin belirlenmesi amacıyla oluşturulmuştur. Retrospektif taramamız esnasında ISS skorunu kullanmak için yeterli dosya bilgisi olmadığı belirlenmiştir. Aynı şekilde, “Comprehensive Research Injury Scale (CRIS)”9, “Trauma Injury Severity Score (TRISS)”10 ve “American College of Surgeons” tarafından oluşturulan bir travma komitesince önerilen “A Severity Characterisation Of Trauma (ASCOT)”11 gibi skorlama sistemlerinin hepsinde de retrospektif taramamız esnasında elde ettiğimiz verilerden daha fazlasına ihtiyaç olduğu görülmüştür. AIS sisteminde belirtilmiş multipl ve lokal travma hastası önermeleri kullanarak yaptığımız multipl ve lokal travma hastası sınıflaması, ISS skoruyla yapılmış olsaydı daha farklı sonuçlar verebilirdi diye düşünmekteyiz. Hastanemiz Acil Servisi’ne başvuran travma hasta grubunun erkek cinsiyet yoğunluğu, ülkemizde yapılmış diğer çalışmalar ile de uyumlu olarak yüksek bulunmuştur3,4. Her ne kadar iş kazaları ve motorlu taşıt kazalarında erkek nüfus travmaya daha fazla maruz kalsa da, kadın cinsiyete sahip olmak, majör travmadan sonra hem fonksiyonel hem de psikolojik problemlerin daha fazla yaşanmasına sebep olan, yaralanma ağırlığı ve mekanizmasından bağımsız bir değişkendir8. Erkekler daha fazla yaralanmasına rağmen kadınlardan daha az sekelle travmayı atlatmaktadırlar. Başvuran hastaların medyan yaşı ülkemizin demografik özelliklerinin ötesinde 3,4 yüksektir . Lokal travma hastaları arasından acil servis monitörlü gözlem odasına veya dış servislere yatırılmayan hastalar, genellikle Acil Servis muayene odalarında endikasyonlarına uygun sürelerde izlenerek taburcu edilmişlerdir. İzole batın travması hastalarının hepsinin acil serviste yatırılarak izlenmesinin nedeni hastane kaynaklarına daha hızlı ve etkin ulaşım nedeniyledir. Hızla hayatı tehdit edici patoloji geçirme ihtimali olan bu tip hastalara acil serviste daha kapsamlı ve iyi bakım verilebilmektedir. Aylara göre travma hastalarının dağılımına bakıldığında ise en yoğun aylar olarak Mayıs ve Ekim ayları belirlenmektedir. TemmuzAğustos-Eylül aylarını içeren yılın 3. çeyreği, anlamlı olarak az hasta gelen zaman aralığıdır (p<0.001). Bunun sebebi olarak, bölgenin sosyoekonomik düzeyinin yüksekliğine paralel şekilde, yaz aylarında İstanbul dışında ikamet düzeyinin yüksekliğinden kaynaklandığını düşünmekteyiz. Ulusal başka çalışmalarda, hastanelerin kendi bölgesel özelliklerine göre farklı zaman dilimlerinde yoğunlaşmalar gözlenmektedir. Ancak en sık hasta görülen aylar birbirine yakındır4. Multipl travma hastalarının kliniklere yatış ve operasyon oranları arasında karşılaştırma yapıldığında, bu grup hastaların çoğunluluğunun acil serviste yatırılarak takip edildiği ve ilgili bölümlerce operasyon endikasyonu konulmasına rağmen acil servisten ameliyathaneye nakledildiği Aylara göre travma oluş mekanizmaları arasında belirgin bir farklılık olsa da istatistikî olarak anlamlı değildir. Bunun sebebi düşük hasta sayısı hem ay hem de mekanizma altına 118 Marmara Medical Journal 2005;18(3);113-122 Haldun Akoğlu ve ark. Daha anlamlı bir sonuca varabilmek için bu hastaların acil serviste bekleme sürelerinin de hesaplanması gerekmekte olup elimizde böyle bir veri yoktur. Multipl travma hastalarının %8,9’u dış servislere, %28,9’ü acil servise yatırılmış olup; acil servise yatırılan hastaların %15’i, dış servislere yatırılanların ise %3’ü aynı gün içerisinde opere edilmiştir. Bu veriler değerlendirildiğinde, multipl travma hastalarının acil serviste izlenip ve buradan operasyona alınmasının tercih edildiği görülmektedir. Bunun sebepleri arasında, yoğun bakım ünitesinde yer sıkıntısı olması dolayısıyla yoğun bakım endikasyonu olan hastaların acil serviste takip edilmesi, hastane katlarındaki izlem ve takip imkânlarının kısıtlılığı, acil servisin bu konuda daha deneyimli ve imkânlarının daha fazla olması, hastaların havayolu girişimlerine ihtiyaç duyması ve bu girişim ve takibi için acil serviste tutulmaları, multipl travma hastalarının birden çok bölümü ilgilendiren durumlarında hangi serviste yatacağı konusunda yaşanan belirsizlik sayılabilir. Yoğun Bakım gerektirmeyen ancak birden çok bölümü ilgilendiren patolojisi olan hastalar için özel takip yataklarının belirlenmesi, bu hastaların takibi ve acil serviste yatış sürelerinin kısalması açısından bir çözüm olabilir inancındayız. yönlendirilmesi, diğer yakın hastanelerdeki motorlu taşıt ve ateşli silah yaralanması hastalarının acil cerrahi gerektiren durumlarının hasta triyajını etkileyerek daha az acilmiş gibi görünen düşme vakalarının önüne geçmesidir. Genç hastalarda, saldırı ve motorlu taşıt yaralanmaları travmatik hasarın en sık sebepleri arasında yer alır. Uluslararası geniş çalışmalarda, özellikle 80 yaşından daha büyük travma vakaları ve bunların mortalitelerindeki artışın, tüm travma vakaları arasındaki sayısal mortalite artışına en çok katkıyı sağlayan yaş grubu olduğu belirtilmiştir2. Genel olarak, yaşlı hastalar genç hastalara oranla gündelik aktiviteleri sırasında yaralanmaya daha eğilimlidirler13,14. Bunun sebebi, hastaların travmaya eğilim yaratan düşkünlük ve yandaş hastalık sayılarında yaşlanmayla beraber gelen artıştır. Türkiye’de 65 yaş ve üstü insan sayısı toplam popülâsyonun %6,7’sini oluşturmasına rağmen (ABD’de %12), hastanemize travma ile başvuran hastalar içinde 65 yaş ve daha yaşlı olanlar %22’lik bir dilimi kapsamaktadır (ABD’de %23)15. 65 yaş ve üzeri travma hastalarını hedefleyen bir çalışmada, bu yaş grubunda en sık görülen travma mekanizmaları sıklık sırasına göre motorlu araç kazası (%45), düşmeler (%35), yaya kazaları (%15), ateşli silahla yaralanmalar (%5) olarak belirlenmiştir16. Düşmelerin çoğu evde ve yer seviyesinde gerçekleşir. Yaşlı hastaların yaşadığı düşmelerin yaklaşık %25’inin altında en az bir dâhili problem yatmakta olduğundan, düşmenin sebebinin değerlendirilmesi en az travmanın ele alınması kadar önemlidir17,18. Düşme ile oluşan kazalarda Acil Tıp doktoru, hasarın ağırlığı ve düşmenin gerçekleştiği yükseklik arasındaki korelâsyona özel önem göstermelidir. 3.5 metre ortalama kat yüksekliği olarak kabul edildiğinde, toplumun %50 ve %90’ı için ölümcül olan yükseklikler (LD50 ve LD90) sırasıyla 4 ve 7. kat seviyeleridir. Alt ekstremite, pelvis ve omurga kırıkları sık görülmekte olup, şok sık bir bulgudur. Diğer künt travma mekanizmalarının aksine, ayaklar veya kalçalar üzerine inişler sonucu oluşan pelvik Oluş mekanizmalarının sıklığı sıralaması klinikler, ülkeler ve yıllar içerisinde belirgin farklılık göstermektedir. 20003 ve 20034 yıllarında, Türkiye’deki başka Acil Tıp Anabilim Dalı bulunan merkezlerden yapılan çalışmalarda en sık sebep düşmeler iken, hastanemizin yakınında bulunan bir merkezden yapılan dört yıllık travma vakaları derlemesinde12, düşmeler sırasıyla trafik kazaları, delici-kesici alet yaralanmaları ve ateşli silah yaralanmalarından sonra ancak 4. sırada gelmektedir. Hastanemizde ise motorlu taşıt yaralanmaları ve ateşli silah yaralanmaları çok az sayıda görülürken, düşmeler çok daha sıktır. Bunun sebebi, düşmeler sonrasında gözlenen kafa travması ile intrakranyal kanamanın sevk zinciri içerisinde ileri, ayrıntılı görüntüleme ve operasyon amaçlı olarak hastanemize 119 Marmara Medical Journal 2005;18(3);113-122 Haldun Akoğlu ve ark. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri fraktürler intraabdominal kanama yerine retroperitoneal kanama oluşturmaya eğilimlidir19. Düşmeler için risk faktörlerine bakıldığında sedatif kullanımı, kognitif bozukluklar, görme yetersizlikleri, inme hikâyesi olması ve osteoartrit varlığı görülür. Ayrıca hastaların kişisel bakımlarındaki bozulma, yaşlanmayla beraber yalnız yaşama insidansındaki artış, reflekslerin yavaşlamasına bağlı vücut kontrolünün azalması, sosyal sebeplerden ötürü bakımevi gibi toplu yaşam alanlarına geçiş ile beraber özbakımda gösterilen hassasiyetin azalması yaşlı hastaların travmaya daha yatkın hale gelmesine neden olmaktadır. Yaşlılık ile oluşan fizyolojik değişiklikler, eş impaksiyon kuvveti altında gerçekleşen hasarın da gençlere nazaran daha fazla olmasına neden olur. Özellikle osteoporoz, hem travmaya eğilimi hem de eş impaksiyon kuvveti altında gerçekleşen yaralanmanın düzeyini belirgin olarak arttırırken; osteoartrit varlığı da hareket zorluğu ve olası travmatik unsurlara verilen refleks yanıtın uygunsuzlaşmasına yol açarak travmaya eğilimi arttırır. Hastanemize başvuran hastalarda ise sıralama düşmeler (%70), delici-kesici alet ile yaralanma (%29) ve motorlu araç kazası (%7) şeklindedir. Ateşli silah ile yaralanma vakaları hastanemize çok nadir yönlendirilmektedir. McGwin ve arkadaşları tarafından yapılan bir çalışmada yaşlı hastaların rekürren yaralanma riskinin daha fazla olduğu gösterilmiştir. Ayrıca deliryum, demans ve hastaneden taburculuk sonrası mortalitelerinin de aynı yaştaki travma dışı hastalardan ve daha genç travma hastalarından daha yüksek olduğu belirlenmiştir20. Geriatrik hastalarda yandaş hastalık ve yaralanma arasındaki bağlantıyı belirlemeye yönelik olarak yapılmış çalışmaların sonuçları birbiriyle çelişmektedir. Sacco ve arkadaşlarının yaptıkları Majör Travma Sonuçları Çalışmasında yaralanma öncesi hastalıkların yaş, anatomik ve fizyolojik ağırlık ile hasar mekanizmasından bağımsız olarak travma sonrası sürviyi etkilediği gösterilmiştir21. Ancak Bross ve arkadaşları ile Oreskovich ve arkadaşları birbirinden bağımsız şekilde travma öncesi var olan hastalıkların sürviyi etkilemediğini belirtmişlerdir22,23,24. 80 yaş ve üzeri travma hastalarını değerlendiren bir başka çalışmada ise komorbiditenin bu yaş grubunda sık olarak görülmesine rağmen, uzun dönem sürvi üzerinde etkili olmadığı belirtilmiştir25. Travma hastalarında önceden var olan yandaş hastalıkları belirleme ve sınıflandırmadaki güçlükler bu farklı sonuçların elde edilmesine neden olmuş olabilir. Her ne kadar yaşlı hastalar yandaş hastalıkları neticesinde daha sık ve tekrarlayan şekilde yaralansalar da, bu yandaş hastalıkları iyi bilinerek uygun tedaviler verildiğinde tedavi oranlarında belirgin farklılık meydana getirmemektedir. Daha yaşlı bir popülâsyondan oluşan düşme hastalarının dâhili problemlerinin sayısının fazla olması, hastalarımızdaki yandaş hastalık prevalansı yüksekliğinin bir sebebidir. Bu hastaların diyabet, demans, serebrovasküler olay, Alzheimer gibi yandaş hastalıklarının ve düşkün durumlarının diğer hasar mekanizmaları ile yaralanan hasta profilindekinden daha fazla olması hastanemize sevk edilmeleri için bir gerekçe olabilir. Ancak yukarıdaki çalışmalar da göstermektedir ki sadece yandaş hastalıklarına göre yaşlı hastaların sürvilerini değerlendirmek yanlıştır ve her hasta kendi içyapısında, kendine has özellikleriyle değerlendirilmelidir. 75 yaş ve daha yaşlı travma hastalarının değerlendirildiği bir çalışmada hastaneden taburcu olabilen hastalarda var olan komorbid patolojiler incelenmiş ve tüm hastalarda en az 2 komorbid hastalık bulunduğu belirlenmiştir26. Bu çalışmada koroner arter hastalığı, hipertansiyon, kronik obstrüktif akciğer hastalığı (KOAH), diabetes mellitus (DM), kanser, demans, inme, gastrointestinal patolojiler ve KBY belirlenen komorbiditeler olarak bildirilmiştir. Kendi hastanemizde de benzer komorbiditeleri tespit etmiş olmamıza rağmen tüm hastalarda bu bilgilere ulaşamadık (Şekil 8). Ancak lokal travma hastalarının multiple travma hastalarına nazaran daha az komorbiditeye sahip olduklarını söylemek mümkündür. Ne yazık ki tüm hastaların ilgili verilerine sahip olmadığımızdan bu açıdan bir genelleme yapmamız uygun olmayacaktır. Son olarak, tartışılması gereken bir diğer konu da 120 Marmara Medical Journal 2005;18(3);113-122 Haldun Akoğlu ve ark. Marmara Üniversitesi Hastanesi acil servisine başvuran travma hastalarının demografik özellikleri hastanelerin travma hacmi ile travma hastalarının mortaliteleri arasındaki ilişkidir. Hastanelerin travma yönetim kapasitesi ve yükü arttıkça travma mortalitesi artabilir. Karşıt görüşlere göre de 1. derece travma merkezi olan hastanelerin hasta bakım başarısı altyapı nedeniyle daha yüksektir. ISS’u 16 ve üzerinde olan, multipl travma hastalarını kabul eden hastaneler birbirleriyle kıyaslandığında; düşük sayıda hastaya bakım veren (240 hastanın altı) merkezlerin mortalitesi %7,62 iken, yüksek sayıda hastaya bakım veren (240 hastanın üzeri) merkezlerin mortalitesi % 5.25 olarak hesaplanmıştır27. Bizim serimizde, bu hesaplamaları yapabilmek için gereken veriler daha önce de belirtildiği gibi yetersiz kalmaktadır. Aynı çalışmayı sonraki yıllar için de tekrarlayarak elektronik kayıt takip sisteminin sağladığı avantajlarla daha sağlıklı bilgiler elde edilebileceğini düşünmekteyiz. 13. This autopsy-based study was performed to assess the relationship between bullous lung disease and smoking in the Turkish population. Methods: One hundred and thirty-one autopsy cases, without any pulmonary injury or known pulmonary disease, and with reliable smoking history, were enrolled. They were divided into two: Group I (non-smokers, n=50, 26 males, and 24 females), and Group II (smokers, n=81, 74 males, and 7 females). Multiple bilateral wedge resections were performed for pathological examination. The relationship between bullous pathology and smoking was investigated. Results: Bullae were found in 6% of group I, and in 40.7% of group II. 32% in group I, and 34.6% in group II had bullous emphysematous pathologies. Overall, 38% in group I, and 75.3% in group II had bullous lung diseases. Bullous lung disease was statistically more common in group II (p=0.087). Conclusion: Even without any respiratory complaint, smoking causes bullous emphysematous pulmonary pathologies. As the incidence of cigarette smoking is high in Turkey, emphysematous changes and bullous lung diseases are expected to be common. Materyal ve metod: Ölüm sebeplerine göre akciğer hasarı olmayan, bilinen akciğer hastalığı bulunmayan ve yakınlarından sigara kullanım öyküleri güvenilir şekilde alınabilen 131 vaka iki grupta incelendi: Grup I (sigara içmeyenler, n=50, 26 erkek, 24 kadın), Grup II (sigara içenler, n=81, 74 erkek, 7 kadın). Bullous lung disease and cigarette smoking: a postmortem study Methods During the the autopsy, lungs of each case were investigated. Localization, size and numbers of macroscopically visible blebs and bullae were noted. Bullae larger than 5 cm in diameter were accepted to be giant. If there were more than one bulla in number, they were named as “multiple”. Then, a wedge resection with part of the neighboring lung parenchyma was performed for all these lesions. In cases where there were no visible blebs or bullae, bilaterally multiple wedge resections concerning apical segments of the upper lobes and superior segments of the lower lobes were performed. Tissue samples were fixed in 10% formaldehyde solution for at least 48 hours, and examined by the same pathologist who was blinded to the cigarette smoking history of the subjects. Analysis: Chi-square, t-test, and Fisher’s exact test were used for the statistical analysis, and p<0.05 was considered to be statistically significant. RESULTS The characteristics of both groups are given in table I. Group I consisted of non-smoking cases (N= 50, 26 males, 24 females, mean age 36±23 years), whereas group II consisted of smoking cases (N= 81, 74 males, 7 females, mean age 43±18 years). Cases in group II had smoked 26±24 pack/years. The causes of death of the cases were listed in table II. For both groups, cranial and abdominal traumas, and gun-fire and penetrating injuries were the most common causes of deaths. INTRODUCTION Bullous lung disease is characterized by the formation of blebs, bullae and emphysema. The term bulla is used to describe an air-filled space within the lung parenchyma resulting from deterioration of the alveolar tissue1,2. Emphysema is a pathologic term that refers to a condition of the lung characterized by abnormal and permanent enlargement of airspaces near the terminal bronchioles accompanied by the destruction of the alveolar walls without obvious fibrosis2,3. Most patients with bullous emphysema are cigarette smokers3-8. The incidence of smoking in the Turkish adult population is around 60-62% in males, and 16-24% in females9,10. Autopsy studies are performed to demonstrate all external and internal abnormalities, malformations, and diseases, and to obtain samples for histological examinations, and any other necessary investigations11. This autopsy-based study was performed in order to assess the relationship of bullous lung disease and cigarette smoking in the Turkish population. METHODS Study Design One thousand-two hundred and fifty autopsies were performed between November 2002March 2003, in the Istanbul Forensic Medicine Institute of the Ministry of Justice of the Republic of Turkey. Cases admitted over 24 hours after death were excluded. Two hundred and ninety-eight subjects who had no pulmonary injury related to the cause of death or no known pulmonary disease were selected. Medical records were examined and the family members were questioned about the smoking history of the subjects. One hundred and sixty-seven cases whose smoking history was unremarkable were excluded, and the remaining 131 cases were taken into the study. The cases were divided into two groups: Group I (non-smokers, n=50, 26 males, and 24 females), and Group II (smokers, n=81, 74 males, and 7 females). Of twenty-six men in group I, 18 (69.2%) had normal pulmonary parenchyma, and 6 (30.8%) had bullous pathology, whereas of 74 men in group II, 18 (24.3%) had normal parenchyma, and 56 (75.7%) had bullous pathology. Concerning the female subjects, of 24 women in group I, 13 (54.2%) had normal pulmonary parenchyma, and 11 (45.8%) had bullous pathology. Of 7 women in group II, 2 (28.6%) had normal parenchyma, and 5 (71.4%) had bullous pathology. Three bullae (6%) were seen in group I, all being single, with a size of less than 5 cm in diameter. Of 33 bullae (40.7%) observed in group II, 17 (52%) were multiple and bilaterally located, 10 (30%) were multiple and unilaterally located. Most bullous lesions (82.1%) were located apically, and in 17.9% of the cases, the superior segments of the lower lobes were affected. Sixteen cases (32%) in group I, and 48 cases (59.3%) in group II had bullous emphysematous pathologies. Overall, 19 cases (38%) in group I, and 61 cases (75.3%) in group II had bullous lung diseases. The bullous lung disease was statistically more common in group II, which consisted of smoking subjects (p<0.0001) Figure 1 shows a sample of the macroscopic appearance of the cut surfaces of apical bullous lesions from two autopsies of male smokers having a small bulla along with mild emphysema, and a large bulla and prominent fibrosis. In the microscopic sections of lung tissue samples of another smoker case, bullous and emphysematous changes (Fig. 2) are shown. The results of the pathological examinations of the cases are given in table IV. Thirty-one cases (62%) in group I, and 20 cases (24.7%) in group II had normal pulmonary parenchyma. DISCUSSION Pulmonary diseases such as chronic bronchitis, emphysema, respiratory bronchiolitis-associated interstitial lung disease are related to cigarette smoking3-8. Emphysema induced by smoking is a common disease in the industrialized part of the world6. Smoking is reported as a causative in the development of bullae4-5. In the postmortem study of emphysema in coal workers and non-coal workers, Cockcroft et al7 reported that emphysema was quite common in postmortem surveys of the general population. They also showed that there was a statistically significant tendency for higher emphysema scores in smokers than in non-smokers among the coal workers7. Sixty-one of the 81 smokers (75.7%) and 19 of the 50 non-smokers (38%) had bullous lung diseases in our study. As was expected, bullous lung disease was statistically more common in cigarette smokers. Patients with bullous diseases can be divided into two groups: those with structurally abnormal lung parenchyma in addition to the bullae and those with normal pulmonary parenchyma1,2. Concerning both groups, we found that 61% of our cases had bullae, 80% had emphysematous lung parenchyma, and 20% had normal lung parenchyma. These findings seem to correlate with the literature. The number of cases having bullae with underlying emphysematous parenchyma is relatively high1,2. Bullae predominate in the upper lobes in smokers4,5. Bilateral involvement is more common in the case of multiple bullae4. Our results were similar to the literature. In our study, 82.1% of bullae were localized in the upper lobes, and in 27 cases of multiple bullae, 63% were bilaterally located. In the Turkish adult population, about 60-62% of males, and 16-24% of females are smokers9,10. In our study, the ratios were 74% in males, and 22.5% in females. Due to that, the prevalence of bullous lung disease was relatively higher in male subjects than in female subjects, but the results were statistically insignificant. Nearly all smokers develop histological evidence of emphysema of various degrees of severity, ranging from minimal to advanced, particularly if they smoke more than 1 pack/day, but only a minority (15%) develops enough physiological dysfunction to become clinically symptomatic3. Eighty-one cases were smokers in our study, and none of them had a known lung disease as reported by their relatives. Cigarette smoking causes the development of bullous emphysematous pulmonary parenchymal pathologies even without any respiratory complaint. Since the incidence of cigarette smoking is high in the Turkish population, emphysematous changes and bullous pathologies of the lungs are expected to be common. In the study of Tylen et al6, high resolution computed chest tomography was performed to investigate the pulmonary parenchyma of 57 smokers and 32 non-smokers. It has been reported that emphysematous changes were detected in 44% of smokers and 3% of nonsmokers. In our study, pathological examination showed bullous lung diseases in 75.3% of smokers and in 38% of nonsmokers. The rate of bullous lung pathology in non-smokers is relatively high in our study. This can be either due to the “secret smoking” for traditional reasons, or to the high incidence of “passive smoking” in the Turkish population. A complete ophthalmological examination, including strabismic evaluation, gonioscopy, ultrasound and magnetic resonance imaging (MRI) was performed. A 14 year-old girl had bilateral nanophthalmos with pseudopapilloedemas and macular folds, anterior chamber angles of Shaffer grade II, and arcuate scotomas. She had orthotropia. There was ipsilateral globe retraction with narrowing of the palpebral fissure on adduction of the left eye, and no ocular motility restriction. Both abducens nerves were detectable on MRI. Her brother had bilateral nanophthalmos without ocular motility disorder. We suggest that the phenomenon of globe retraction in this case may be related to mechanical factors rather than innervational abnormalities. Hastaya şaşılık muayenesi, gonyoskopi, ultrasonografi ve magnetik rezonans görüntülemesini (MRG) de içeren kapsamlı göz muayenesi yapıldı. On dört yaşındaki kız hastada psödopapilödem, maküla kıvrımları, Shaffer II derecesinde ön kamara açısı ve arkuat skotom ile birlikte nanoftalmus vardı. Olgu ortotropik ve göz hareketleri serbest idi. Sol gözün addüksiyonunda ipsilateral glob retraksiyonu ile birlikte palpebral fissür daralması görülmekteydi. MRG’de her iki abdusens siniri belirgindi. Kardeşinde de, göz hareketlerinde bozukluk olmadan, bilateral nanoftalmus vardı. Bu olgudaki glob retraksiyonunun innervasyonel sebeplerden çok mekanik faktörlere bağlı olduğunu düşünmekteyiz. Globe retraction and narrowing of the palpebral fissure on adduction are typical components of the Duane’s retraction syndrome (DRS). Numerous systemic and ocular disorders have been reported in association with DRS3. We report a case of bilateral nanophthalmos with ipsilateral globe retraction and narrowing of the palpebral fissure on adduction of the left eye. INTRODUCTION The characteristic features of nanophthalmos are narrow palpebral fissures in combination with a deep set globe in a small orbit, hyperopia, short axial length, reduced volume of the globe with normal lens volume, and normal or reduced corneal diameter1. Globe retraction in a patient with nanophthalmos healthy 11 year-old boy without a history of abnormalities during pregnancy or birth, had bilateral nanophthalmos without any ocular motility disorder. Mother and father were secondary degree relatives. CASE REPORT A 14 year-old girl with normal prenatal and natal anamnesis, and no history of disease, had bilateral narrow palpebral fissures and deeply set globes (Fig. 1). Best corrected visual acuity was 0.4 (+ 13.0) OD, and 0.5 (+ 11.0) OS. Horizontal corneal diameters were 11.0 mm bilaterally, and the irises were thick with a convexity towards the cornea. Gonioscopy revealed bilateral anterior chamber angles of Shaffer grade II, peripheral anterior synechiae and prominent iris convexity. Intraocular pressures were 11 mm Hg in both eyes. Visual field testing showed bilateral arcuate scotomas, more extensive in the right eye. There were bilateral pseudopapilloedemas and macular folds. Orbital B-scan ultrasonography revealed small globe diameters, slightly small lens diameters, shallow anterior chambers, and thick scleras. MRI showed bilateral normal abducens nerves and symmetrical orbital anatomy. There was orthotropia in primary position and no restriction of ductions and versions. Diplopia, head turn or nystagmus were not observed. On adduction of the left eye, there was ipsilateral globe retraction and narrowing of the palpebral fissure (Fig. 1). DISCUSSION Our case showed the typical features of nanophthalmos, and globe retraction with narrowing of the palpebral fissure of the left eye on right gaze. Globe retraction and narrowing of the palpebral fissure on adduction are typical components of DRS, and have been related to the paradoxical innervation of the lateral rectus muscle, leading to co-contraction of the horizontal rectus muscles3. In DSR magnetic resonance imaging can show absence of the abducens nerves4. Our case had no other features of DRS, and both abducens nerves were present. Gross et al.5 report a case of DRS type II, with synchronous innervation of the ipsilateral horizontal rectus muscles without globe retraction. They propose that in DRS both innervational and mechanical factors are involved. In our case, we propose a mechanical factor for the globe retraction, though it remains interesting why despite symmetrical anatomy, globe retraction was observed in only one eye of the patient. CASE REPORT HOT WATER EPILEPSY (HWE) FAMILIAL AND NONFAMILIAL CASE REPORTS İpek Midi, Kadriye Ağan, Canan Aykut- Bingöl Department of Neurology, School of Medicine, Marmara University, İstanbul, Türkiye ABSTRACT Reflex epilepsy is a form of epilepsy precipitated by a specific stimulus and events. Seizure precipitation by hot water during bathing is named as “hot water” or “bathing epilepsy” which is a rare and unique form of reflex epilepsy. It is especially common in Southern India and of higher frequency in Muslims among them. There are also isolated and rare case reports from Australia, Japan, Canada and the United States. A small series of patients with hot water epilepsy (HWE) was reported from Turkey. The cases with HWE may be sporadic or familial. Our aim is to outline the clinical and electroencephalographic (EEG) features of four patients with hot water epilepsy, seen at our clinic. One of our patients has a family history of epilepsy. The second and third generation of family members had seizures during bathing. It is important to ask patients with HWE about bathing habits (pouring hot water over the head from a bowl), the temperature and amount of bathing water, the duration of bathing, any additional spontaneous seizures and family history. Sometimes this is enough to eliminate precipitating factors for treatment. Keywords: Reflex epilepsy, Hot water, Pathophysiology, Treatment SICAK SU EPİLEPSİSİ AİLESEL VE SPORADİK OLGU SUNUMLARI ÖZET Refleks epilepsi iç ve dış uyaranlar veya etkenlerle oluşan bir epilepsi türüdür. Banyo sırasında sıcak su ile tetiklenen nöbetler “sıcak su” veya “banyo epilepsisi” diye adlandırılır ki refleks epilepsinin nadir rastlanan bir formudur. Özellikle Güney Hindistan bölgesinde yaygın olup, Müslümanlar arasında sıklığı daha fazladır. Bunun dışında Avustralya, Japonya, Kanada ve Amerika’da da izole, nadir olgular bildirilmektedir. Ülkemizede de sıcak su epilepsine ilişkin küçük bir seri mevcuttur. Sıcak su epilepsisi sporadik veya ailesel özellik gösterebilir. Bu çalışmada polikliniğimizde takip edilen sıcak su epilepsili 4 olgunun klinik ve elektroensefalografik (EEG) özelliklerini ortaya koymak amaçlanmıştır. Hastalarımızdan birinde aile hikayesi de bulunmaktadır. Hastanın 2. ve 3. kuşak akrabalarında banyo yaparken nöbet geçirme söz konusudur. Sıcak su epilepsisi olan hastalara banyo yapma alışkanlıkları (sıcak suyu bir tas ile başlarından aşağı dökme gibi), suyun sıcaklığı, banyoda kalma süresi, sıcak su ile tetiklenen nöbetlerin dışında spontan nöbetlerin olup olmadığı, aile hikayesinin bulunup bulunmadığının sorulması önemlidir. Bazen tedavide sadece tetikleyen faktörlerin ortadan kaldırılması yeterli olmaktadır. But when I am taking a hot bath in a short time or with warm water, I do not have any problem. And also, when I have a shower without pouring water directly on my head, I have no seizure, either”........ INTRODUCTION Reflex epilepsy is a form of epilepsy provoked by a specific stimulus or event. It is only 6% of all epilepsies1,2. Reflex epilepsy is a collection of different seizure types and their specific provoking stimuli2. Photosensitive epilepsy (induced by light stimuli) is the most common type2. Other than this, seizures induced by thinking, reading, eating, shocks, music or hot water are also classified as reflex epilepsy1,2. Some of his second and third generation of family members have seizures during bathing. His neurological examination, MRI and interictal EEG were all normal and the seizures were controlled when the provocating factor was eliminated. Seizure precipitation by hot water during bathing is named “hot water” or “bathing epilepsy” which is a rare form of reflex epilepsy. I.M.Allen, in 1945, from New Zealand first described a 10-year-old boy who had seizure during bathing. Other case reports followed from Japan, Canada, Australia, and the United States. The largest series, 279 patients, have been reported from Southern India. Although the percentage of patients with HWE was 0.6% among all patients with epilepsy, in Southern India HWE patients were reported as 3.6-3.9%3-5. Case Report 2 An 18-year-old female patient has seizures while having a bath. .............”While my mother was bathing me with hot water, I was getting a pleasant feeling when I looked at one point on our bathroom’s wall. And then I had been turning around myself unpurposely. After 7 years old, I realized that when I did not look at a point during bathing I did not have seizures. But when I was 14 years old, looking at a point during bathing gave me a pleasure. So I kept looking at one point on the wall to live this experience again and again”.............. It is possible to provoke the seizure by pouring hot water over the head in the bath. The temperature of water used ranges between 40-50ºC and the room temperature is 25-30ºC4,5. The majority of patients are male and children. A positive history of epilepsy among family members has been reported in 7-20% of cases. Spontaneous non-reflex epilepsy is reported in 25% of patients4,6. The pattern of epileptic seizure is generally complex partial form but generalized tonicclonic seizure (GTCS) is also reported3. This patient’s MRI, neurological examination and interictal EEG were also normal. Case Report 3 An 18 year-old male patient presented with history of seizure while taking a bath with hot water since the age of three. He used to become motionless with activity arrest. He explained that the precipitating factors were the water’s temperature and pouring it directly over his head. CASE REPORTS Case Report 1 A 23-year-old male patient reported to have a GTCS when he took a bath with hot water especially when he spent a long time in the bath. His interictal EEG showed mild and generalized disorganized background. Interictal EEG is generally normal but in 20-25% of cases show diffuse abnormalities. The factors that provoke seizures are the temperature and the amount of water, the body surface of pouring water, the type and the duration of bathing3. Case Report 4 A 19 year-old female patient admitted to our hospital for the first episode of seizure while she was taking a bath. ..............”After the dinner I was taking a shower, but the water was hot and the time I spent was about 10 minutes. I felt something getting wrong and I decided to open the window to breath comfortably. And I did not remember anything after that. It was the first episode I experienced. My mother found me in the bathroom in a tonic-clonic movement with a ‘ cyanotic appearance’. In our cases the main precipitating factors for seizures were bathing with hot water and pouring water over the head. It seems that traditional factors are important here, because Turkish people have a bath by sitting and pouring water with a bowl3. In some patients, seizures are only activated by hot water, but spontaneous non-reflex epilepsy is also reported in different series from 16-38% to 62% of patients3,4,6. Twenty-five percent of cases develop nonreflex epilepsy within 1-3 years9. The patient’s neurological examination and MRI were normal. Five to ten percent of patients had seizures even during a body bath, when water is not poured overhead. Eleven to twenty-seven percent of patients had febrile convulsions prior to the development of reflex epilepsy. A positive history of epilepsy among the family members had been reported in 7-20% of cases4,6. Her sleep deprivation EEG showed epileptiform activity and she was advised to take bath with warm water. Valproic acid (VA) treatment was begun for the control of her seizure. DISCUSSION Hot water epilepsy refers to a specific type of reflex epilepsy. HWE is considered to be a geographically specific epileptic syndrome since it mainly occurs in Indian7. HWE is mostly seen in the first decade of life. It is more common in children, with cases more frequent among male than female patients (70%)4. The seizure could occur either at the beginning or at the end of the bath and last about 1 to 3 minutes. The pattern of epileptic seizure, which is seen in HWE, consists of 67% complex partial (CPS) and 33% generalize tonic clonic seizure (GTCS). In a different study CPS was reported in 40%, GTCS in 60%3,4. Gururaj G, et al., noticed that the frequency of seizure occurrence was more than 1-4 attacks/month in 89% of cases8. Symptoms onset included a dazed look, a sense of fear, irrelevant speech, and visual and auditory hallucinations with complex automatism4. The mechanism of reflex epilepsy is not clear. A defect of thermoregulation center in the hypothalamus might be the underlying mechanism of reflex epilepsy. When a hotwater epileptic patient takes a bath, his body temperature increases 2-3ºF in the short span of 2 minutes and returns to the baseline after 10-12 minutes when the bathing is completed. On the other hand, when normal healthy volunteers have a bath, the body temperature raises 0.5 to 0.6ºF and returns to baseline immediately at the end of the bath. So, HWE patients probably have an aberrant thermoregulatory system and are extremely sensitive to the rapid rise in temperature. This aberrant thermoregulation system seems to be genetically determined4. The cases with HWE may be sporadic or familial. In familial type, the transmission of inheritance is not known. In our clinic, we have encountered one patient with a positive family history of HWE . REFERENCES 1. 2. Patients with HWE reported feeling pleasure during their seizure and nearly 30% of them triggered their seizure by themselves1-3. Case 2 has also mentioned this feeling of pleasure. HWE is generally known as benign and selflimited, but sometimes antiepileptic drugs are necessary for controlling seizures. Avoidance from hot water may be sufficient to be seizure free. If it is necessary to take a medication,VA and carbamazepine (CBZ) might be preferred. Rectal diazepam (DZP) may also be given 30 minutes before bathing3,6. 3. 4. 5. 6. Some of our patients are still not taking any medications. They are taking their bath in a shorter time and with warm water. 7. HWE is a benign form of reflex epilepsy. Ponksiyon sırasındaki dural zedelenme nedeniyle devam eden beyin omurilik sıvısı (BOS) sızıntısı, meningiyal damarlarda gerilme ve yırtılma ile kanamaya yol açmaktadır. Yazımızda genel anestezi altında “burr-hole” ile dekompresyon operasyonuna aldığımız, 10 gün önce spinal anestezi denenen, başarılı olmayınca genel anestezi altında sezaryen operasyonu geçiren, hastanemize başvurduğunda postspinal bilateral intrakranyal subdural hematom teşhisi konan olguyu literatür bilgileri ışığında tartışmayı amaçladık. Olgu: On gün önce sezaryen operasyonu geçiren 35 yaşındaki kadın olgunun operasyon sonrası konservatif tedavilere yanıt vermeyen baş ağrısı şikayeti olmuş. Şiddetli baş ağrısı ile acil servise başvuran hastaya subdural hematom tanısı konmuş ve konvülziyon geçirmesi üzerine acil şartlarda genel anestezi altında dekompresyon operasyonuna alındı. Tartışma ve Sonuç: Başarısız bile olsa spinal anestezi denemelerinden sonra, sebat eden baş ağrısı varlığında subdural hematom olabileceği düşünülerek, dikkatli öykü alınmalı, radyolojik görüntüleme tetkikleri ile erkenden tanı konmalıdır. Baş ağrısını gidermede verilen analjeziklerin tabloyu maskeleyebileceği hatırda tutulmalıdır. Anahtar Kelimeler: Spinal anestezi, Subdural hematom, Nörolojik komplikasyon POST-SPINAL BILATERAL INTRACRANIAL SUBDURAL HAEMATOMA (CASE REPORT) ABSTRACT Background and Aim: Post-spinal intracranial subdural haematoma (ISDH) is very uncommon, has serious complications and can even be mortal if it is not treated. Cerebrospinal fluid (CSF) leakage which originating from a dural puncture may cause haemorragia by exraction of the meningeal vessel. In this case report, we aimed to report a bilateral intracranial subdural haematoma case who had had failed spinal anesthesia ten days previously, and discuss the case in the light of the literature. Case: A thirty-five-year old woman diagnosed with acute subdural haematoma was operated. The patient had had a Caesarean section ten days previously. After that she had a headache which did not answer to conservative treatment. The patient was diagnosed to have a subdural haematoma, and an emergency decompression operation was performed under general anesthesia. Discussion and Conclusion: Subdural heamatoma complication should be considered in case of consistent headache after spinal anesthesia which may have failed and a careful examination and radiological evaluation must be made. The analgesics used to treat the headache may veil clinical appearance and this must be kept in mind. Postspinal bilateral intrakranyal subdural hematom (olgu sunumu) ilaç kullanımı öyküsü olmayan olgunun sezaryen operasyonu sonrası derlenmeyi takiben şiddetli baş ağrısı olduğu, istirahat, analjezik ve bol sıvı almakla baş ağrısının hafiflediği belirlendi. Sezaryen operasyonu için rejyonel anestezi denenip denenmediği sorgulandığında operasyon öncesi hastaya birkaç kez spinal ponksiyon denendiği ancak başarılı olmayınca genel anesteziye geçildiği anlaşıldı. Operasyon sonrası 3. gün eve taburcu edilen hastanın, baş ağrısının şiddeti değişmekle beraber devam etmesi üzerine özel bir kliniğe başvurduğu, kraniyal manyetik rezonans görüntüleme (MRG) ile sol SDH saptandığı anlaşıldı. Fakültemiz acil servisine başvuran hastanın baş ağrısının şiddetlenmesi ve konvülziyon geçirmesi üzerine bilgisayarlı tomografi (BT) ile kraniyal görüntülemenin (Şekil 1) ardından acil dekompresyon operasyonu planlandı. GİRİŞ Spinal anestezinin en yaygın komplikasyonu baş ağrısıdır. Dura zedelenmesine bağlı gelişen baş ağrısı genellikle postural olup, sıvı alımı ve yatak istirahatı ile ortalama 48 saat içerisinde geçmektedir. Uzun süren baş ağrısı ise subdural hematom (SDH) veya intrakraniyal kanamadan kaynaklanabilir1-9. Spinal anestezi sonrası dural zedelenmeye bağlı beyin omurilik sıvısı (BOS) sızıntısı fazla olduğunda beyin kaudale doğru yer değiştirmektedir. Bu yer değişikliği sırasında menengiyal damarlarda gerilme nedeniyle kanama bildirilmektedir10. Spinal anestezi sonrası tek taraflı SDH oldukça iyi tanımlanmakla birlikte bilateral subdural hematom (BSDH) gelişimine ait çok az sayıda yayın vardır11. Literatürde başarısız spinal anestezi denemesinden sonra intrakranyal BSDH gelişimine ait bir bilgiye ise rastlamadık. Bu nedenle sezaryen operasyonunu gerçekleştirmek amacıyla spinal anestezi denenen, ancak ponksiyon başarılı olmayınca genel anestezi altında operasyonu tamamlanan ve operasyon sonrası şiddetli baş ağrısı ve BSDH gelişen olguya ait deneyimlerimizi sunarak literatür bilgileri ışığında tartışmayı amaçladık. OLGU SUNUMU Acil serviste SDH tanısı ile izlenen 35 yaşındaki kadın hasta bilinçte kapanma ve tonik-klonik yaygın konvülziyon gelişmesi üzerine genel anestezi altında acil dekompresyon operasyonu için değerlendirildi. Konvülziyon sonrası uykuya eğilimi olan ve yeterli kooperasyon sağlanamayan olgunun öyküsü yakınlarından alındı. Şekil 1: Preoperatif kraniyal BT Hastanın preoperatif muayenesinde bilinci uykuya eğilimli, pupiller izokorik, ışık reaksiyonu bilateral müsbet, kan basıncı:120/80 mmHg, kalp atım hızı:85/düzenli, biyokimya tetkikleri normal klinik sınırlar içinde saptandı. Acil şartlarda operasyon odasına alınan olguya 2 dakika süreyle preoksijenizasyon uygulandı. Anestezi indüksiyonu iv 5 mg/kg sodyum tiyopental ile gerçekleştirildi. Kas gevşekliği Son üç yıldır guatr tanısıyla takipte olduğu, demir eksikliği anemisi nedeniyle gebeliğinde demir preparatı (Ferro Sanol Duedonal kapsül. Melusin®) kullandığı, on gün önce genel anestezi altında sezaryen operasyonu geçirdiği öğrenildi. Gebeliği boyunca düzenli takipleri yapılan, travma, koagülopati, ciddi enfeksiyon veya anti-koagülan ya da bitkisel 136 Marmara Medical Journal 2005;18(3);135-139 Berrin Işık ve Ark. Postspinal bilateral intrakranyal subdural hematom (olgu sunumu) etkidir 12. Dural zedelenme nedeniyle günde 250 mL den fazla olan BOS kaçağının baş ağrısına neden olduğu düşünülmektedir13. Dural zedelenme sonrası BOS'un kaybı intrakraniyal yapıların kaudal yöne yer değiştirmelerine neden olur. Yer değiştirmenin yarattığı gerilim ise dura, sinüs ve kan damarları gibi duyarlı dokularda ağrıyla sonuçlanır. Post-dural zedelenmeye bağlı baş ağrısı genellikle dural zedelenmeyi takiben başlar, analjezik ve bol sıvı alımının yanı sıra yatak istirahatı ile birkaç günde geçer. Daha çok frontal, oksipital alanda görülür, boyun ve omuzlara da yayılabilir. Öksürmek, ayağa kalkmakla şiddetlenir. Bazen bulantı kusma, depresyon ve iştah kaybı da eşlik eder. Gençlerde ve kadınlarda daha sık görülür14. Post-dural zedelenmeye bağlı baş ağrısının şiddetlenmesi veya uzaması ise SDH veya intrakranyal kanamanın göstergesi olabilir1-9. BOS'un ani kaybı ile beynin kaudale yer değiştirmesi sırasında serebral köprü venlerinin gerilmesi ve kopması kanama ile intrakranyal SDH'ya neden olabilir10,15. Thorsen16 spinal anestezi sonrası beyin yüzeyinde birden çok peteşiyal kanama olduğunu tanımlamıştır. Serebral atrofi, dehidratasyon, ventriküler şant varlığı, antikoagülan kullanımı, kanama eğilimi SDH'yi kolaylaştıran nedenlerdir. Subdural hematom varlığında baş ağrısı uzun sürer, pozisyonel değildir ve konvansiyonel tedavilere fazla cevap vermez. İşitme kaybı, diplopi veya bilinç kaybı eşlik edebilir1-9,14,15. Macon ve ark.17 SDH'ya bağlı baş ağrısının post-dural delinmeye bağlı baş ağrısına göre (yatak istirahatına rağmen) daha uzun ve şiddetli seyrettiğini bildirmektedir. Suess ve ark.18 myelografi sonrası intrakranyal hemoraji gözlenen 17 olguyu değerlendirdikleri çalışmalarında temel bulgu olan baş ağrısının 5 günden fazla devam ettiğini bildirmektedir. Diğer taraftan Thoennissen ve ark.19 16 klinik çalışmayı içeren metaanaliz sonucunda; postdural zedelenme baş ağrısı görülme sıklığında erken mobilizasyon ve kısa yatak istirahatı yapanlar ile uzun yatak istirahatı yapanlar arasında anlamlı fark olmadığı kanısına varmıştır. Bu nedenle sebat eden baş ağrısı varlığında 0.5 mg/kg rokuronyum bromid ile sağlandı. Yeterli anestezik derinliğe ulaşıldığında hasta orotrakeal entübe edilerek anestezi %50/50 O2/hava içerisinde %0.6-1 izofluran inhalasyonu ve iv 0.5 µg/kg/dk remifentanil infüzyonu ile sürdürüldü. İşlem boyunca periferik damaryolundan 5 ml/kg/saat Ringer Laktat solüsyonu verildi. Elli dakika süren “burr-hole” ile dekompresyon operasyonunun ardından remifentanil infüzyonu ve izofluran inhalasyonu sonlandırıldı. Önce %100 O2 ardından %50 O2 %50 hava ile solutulan hasta spontan solunumunun geri dönmesi üzerine ekstübe edilerek yoğun bakım servisine çıkarıldı. Kontrol BT ardından (Şekil 2) yoğun bakımda 4x4mg iv dekzametazon (5mg/5mL Dekort Ampul® ) ve oral 3x1 fenitoin sodyum (Epanutin 100 mg kapsül Pfizer®) tedavisi başlandı. Postoperatif 2. gün genel durumu iyi, bilinci açık olan hastaya monitorize hasta bakımı altında kontrol kraniyal MRG yapıldı. Görüntüleme sonucunda sağ tarafta da 5x5mm SDH saptandı. Ancak cerrahi planlanmayarak, spontan rezorbsiyona bırakıldı. Postoperatif onuncu gün şifa ile hastaneden taburcu edildi. Bir ay sonra yapılan kontrolde de herhangi bir sorun saptanmadı. Postspinal bilateral intrakranyal subdural hematom (olgu sunumu) nörolojik kompikasyon geliştiğini düşünerek radyolojik tetkik yapmak doğru bir karar olacaktır. Olgumuzda spinal anestezi denemesi ardından uygulanan genel anestezi sonrası baş ağrısı gelişmiş, analjeziklerle maskelenmekle beraber tümüyle kaybolmamış hatta daha da şiddetlenmiştir. Onuncu günde çekilen kraniyal görüntüleme ile SDH saptanmasından kısa bir süre sonra da bilinç kapanarak konvülziyon gelişmiştir. Dural ponksiyon sonrası tek taraflı SDH oldukça iyi tanımlanmakla beraber BSDH'ya ait çok az bilgi vardır11,20,21. Baldwin ve arkadaşlarının21 BSDH saptanan 15 olgudan 12'sinin obstetrik olgular olduğunu bildirmesi ise dikkat çekicidir. Antikoagülan ilaç kullanımı, kanama eğilimi, travma, yaşlılık, beyin atrofisi, serebral anevrizma, beyin tümörü, serebrovasküler olaylar ve meningo-vasküler sifiliz gibi durumlar SDH görülme sıklığını artırmaktadır1-9,13,14. Sezaryen nedeniyle spinal anestezi denenen olgumuzda herhangi bir risk faktörü saptanmamıştır. Genel anestezi sonrası gelişen baş ağrısı nedeniyle hasta konvansiyonel analjezikler kullanmış ve yatak istirahati yapmıştır. Post-dural zedelenmeye bağlı baş ağrısını etkileyen faktörlerden biri de rejyonal anestezi tekniklerine ait farklardır. Geleneksel keskin uçlu ve geniş çaplı iğnelerle baş ağrısı görülme sıklığı artarken, kalem uçlu ve 29G iğnelerle azaldığı bildirilmektedir15. Öte yandan ince iğne ile yapılan spinal anestezi sonrası SDH geliştiğine dair olgular da vardır5,22. Sunduğumuz olguda spinal anestezi girişimi bir başka klinikte yapıldığından iğne özelliklerini bilemiyoruz. Ancak anestezi kayıtlarından spinal ponksiyonun başarılı olmadığı bu nedenle genel anesteziye geçildiği, genel anestezi sırasında da herhangi bir komplikasyon yaşanmadığını öğrendik. Postdural zedelenme başağrısının tedavisinde; subaraknoid alandan BOS sızıntısını önlemek amacıyla erken dönemde epidural alana kan yaması yapılması veya oral 10mg/kg kafein verilmesi önerilmektedir2,23. Ancak Davies ve ark.24 iyatrojenik dural ponksiyon sonrası 36. saatte epidural kan yaması yapılmasına rağmen, dural ponksiyondan tam 14 gün sonra SDH tanısı konularak opere edilen obstetrik bir olgu bildirmiştir. Olgumuzda sezaryen operasyonu için denenen spinal anestezi başarıyla gerçekleştirilemediğinden genel anesteziye geçilmiş, dural zedelenme fark edilmediğinden bu yönde bir tedavi yapılmamıştır. Anesteziden derlenme sonrasında şiddeti değişmekle beraber baş ağrısı sebat etmiş, sol intrakraniyal SDH gelişimi postoperatif 10. günde saptanmıştır. Sol SDH'nin kafa içi basınç artışı bulguları vermesi ve konvülziyon gelişmesi üzerine de acil cerrahi müdahale uygulanmıştır. Daha sonra gelişen sağ intrakraniyal SDH ise konservatif yöntemle tedavi edilmiştir. Sonuç olarak spinal anestezi girişimleri ardından intrakraniyal SDH gözlenebilmektedir. Farmakolojik tedavi ise nörolojik belirtileri maskeleyerek tanının gecikmesine yol açabilir. Bu nedenle spinal anestezi girişimi sonrası sebat eden baş ağrısı varlığında dikkatli öykü alınmalı, detaylı nörolojik ve radyolojik değerlendirme yapılmalıdır. Özellikle spinal anestezi denenen olgularda anestezistlerin yanı sıra hastanın klinik takiplerini sürdüren kadın doğum uzmanları, baş ağrısı yakınmasını değerlendiren nöroloji ve beyin cerrahisi uzmanları postspinal intrakranyal SDH konusunda dikkatli olmalıdır. INTRODUCTION Primary retroperitoneal lipomas are rare benign tumors that usually present as an abdominal mass or with patient complaints of pressure symptoms to adjacent organs. Histologically, lipomas originate from mature adipose tissue of the retroperitoneum, mesentery, or the gerota fascia.1,2 Retroperitoneal lipomas must be carefully differentiated from liposarcoma of low-grade malignancy, in order to provide the correct treatment and postoperative follow-up3 . A low grade liposarcoma is difficult to differentiate from a benign lipoma based solely on CT scan or MRI findings, but heterogeneity, areas of enhancement or necrosis, and irregular margins are often seen on the CT scan of a liposarcoma.4 CASE REPORT A sixty-one year old otherwise healthy woman presented with persistent left flank pain of one year duration. She also reported a vague sensation of an abdominal mass. She had no history of weight loss, fever, gastrointestinal symptoms, or change in bowel habits. Her past medical history was significant only concerning hypertension, and a cholecystectomy 20 years previously. Physical examination findings revealed fullness in her left lower quadrant. Abdominal ultrasonography (USG) revealed the presence of voluminous hypoechoic homogenous retroperitoneal tumor that displaced the left colon anteriorly and medially. Intravenous urography showed a homogenous radiolucent mass in the left upper quadrant, which displaced small bowel loops and the left kidney (Fig. 1). A giant retroperitoneal lipoma: a case report imaging (MRI) of the abdomen and pelvis demonstrated a large, well-circumscribed, homogeneous mass anterior to the left iliopsoas muscle extending from the inferior pole of the left kidney to the pelvis (Fig. 2). The left colon and small bowel were displaced medially. bloc. The patient recovered uneventfully and was discharged home on postoperative day 6. Grossly the mass was 30x26x17 cm and weighed 3490 gr (Fig. 3). On cut section, lipomatous areas with focal hemorrhage were detected. The cut surface of the tumor was almost completely uniform light-yellowish color. The histopathological examination showed typical adipose cells of mature appearance, laid out in a compact arrangement, sometimes with intervening septa of vascular collagen tissue, without signs of malignancy (Fig. 4). A giant retroperitoneal lipoma: a case report Almost all reported retroperitoneal tumors were easily and completely resected, without any invasion of adjacent structures. Total excision is the treatment of choice. DISCUSSION Lipomas are the most common benign tumors of the adipose tissue among adults.5 According to histopathological findings, they are subclassified into conventional lipoma, fibrolipoma, angiolipoma, fusiform cell lipoma, myelipoma and pleomorphic lipoma.5 Retroperitoneal lipomas are extremely rare, slowly growing benign tumors of adipose tissue. Microscopically, lipomas consist of multivacuolated cells, small esonophilic cells, and univacuolated adipocytes.6 Classic lipomas have CT and MRI signal characteristics similar to subcutaneous fat (between -65 and -120 Hounsfield units).2 Magnetic resonance imaging will reveal an intense signal on T1-weighted images.7 Previous use of angiography for lipomas showed the tumors to be hypovascular.7 REFERENCES 1. 2. 3. 4. 5. 6. Although retroperitoneal lipomas are relatively more common in adults, they can occur in infants and small children.8 They may affect both sexes, but there is a greater predisposition for females. 7. 8. 9. As well as being a large and growing clinical problem, it consumes a considerable proportion of health care resources in both developed and developing countries. Ideally, every ESRD patient should have access to dialysis. The reality is that there is not enough money for healthcare in the developing world, particularly for expensive and chronic treatment such as RRT. Ninety percent of them live in the developed countries or, as defined by the World Bank (WB), high-income countries where the average gross income is in excess of $ 10.000 per capita2. improved survival from other diseases (so called competitive risk) and wider acceptance criteria for RRT, more elderly patients, patients with diabetes, and patients with several other co-morbidities (malignancies, systemic diseases etc) requiring RRT10. The most obvious trends are the rapid increase in RRT incidence in the older patients and in patients with diabetes mellitus. These changes are due in large part to an increase in these population groups in the community. However, improved access to RRT will also have contributed to the increase in incidence observed. In the epidemiological data of treated endstage renal failure in the European Union (EO) during the year 1995, the mean incidence of new patients was 120 pmp with values ranging from 68 in Finland to 163 in Germany. These numbers are much higher in USA with 262 pmp and Japan (210), but lower in Canada (140) during the same year3. According to analysed data from nine countries participating in the ERA-EDTA registry (Austria, Belgium, Denmark, Finland, Greece, The Netherlands, Norway, Spain, UK (Scotland)), the adjusted incidence rate of RRT increased from 79.4 pmp (range: 58.4101.0) in 1990-1991 to 117.1 pmp (range: 91.6-114.8) in 1998-1999, ie 4.8% (3.1-6.4) each year in Europe11. This increase was greater in men than in women, 5.2 vs 4.0 %/year. In most countries, the incidence rate remained stable for those younger than 45 years, rose by 2.2% on average in the 45-64 years age group and by 7.0% among those 6574 years and tripled over the decade in those 75 years old or older. The mean age of the ESRD population increased 3.5% between 1995 and 1998, to 62.2, in US12. During 1996, 283,932 patients were treated for ESRD and 73,091 new patients started ESRD treatment in the USA4. Incidence and prevalence rates tend to increase with age. The largest group of patients falls in the 45 to 64- year age group. The disease was more common in men than women. Black race constituted 30% to 32% of treated ESRD patients in contrast to 12.6% of the US population. In 1998, the incidence of treated ESRD in Europe ranged from 110 pmp in the Netherlands to 192 pmp in Germany5. Even higher incidence rates were recorded in the same year in countries outside Europe, such as the USA (>300 pmp) and Japan (200 pmp)6. In 1998, the prevalence of treated ESRD in Europe ranged from 498 pmp in the UK to 854 pmp in Italy5. Again, Japan and the USA recorded higher rates than European countries, with prevalences of >1400 pmp and almost 1200 pmp, respectively6. 1.2.2. Etiology of ESRD The major causes of ESRD in North America and in many developed countries are diabetes and hypertension, which together account for almost 60% of dialysis patients. In the USA, diabetes is the most common attributed cause of ESRD, followed by hypertension, glomerulonephritis, others (interstitial nephritis, vasculitis, eg), unknown, cystic diseases and urological diseases4. In the youngest group (<20 years), the most common diagnoses are glomerulonephritis (31.7%) and cystic/hereditary/congenital diseases (24.4%), whereas diabetes is rare. For the oldest age group (64 years) the most common attributed causes of ESRD are A total of 1764 people commenced RRT in Australia in 2000, a rate of 92 pmp, in New Zealand, 417 people commenced RRT in 2000 (107 pmp)7. The Australian and New Zealand rates are substantially lower than the USA (311 pmp)8 and Japanese (240pmp) but similar to the rates reported from England (89 pmp) and most European countries9. The incidence and prevalence of RRT for ESRD has continued to increase through the world, but at rates that vary considerably between countries. Diabetes is relatively more common in women and hypertension is relatively more common in men. the US (41.8 pmp) (11). Diabetes was a cause of ESRD in 28.8% of German patients who survived more than 90 days on HD. The percentage was considerably lower in Italy, Spain, and the UK (between 20.1 and 21.7%) and only 15.3% in France14. Racial differences exist in the distribution of patients by primary renal disease. Diabetes is especially common among Native Americans (63.2%) and, to a lesser extent, Asians/Pasific Islanders (41.6%). Hypertension is notable high among blacks (35.9%) and glomerulonephritis is disproportionately high for Asians-Pacific Islanders (17.8%) that is twice as frequent as in black patients. It is unclear why the rate of diabetes continues to increase even when adjusted for age, gender, and race, while rates of nondiabetic incidence remain steady. While low protein diets, blood pressure control, and ACE inhibitors have all been available for some time, they do not appear to have influenced the diabetic populations. The dramatic rise in the incident dialysis population may be related to advancing obesity, carbohydrate intolerance and insulin resistance12. In 1998, the incidence of ESRD was 133 pmp for diabetes, 71 pmp for hypertension, 38 pmp for glomerulonephritis, 15 pmp for urological disease, 9 pmp for cystic kidney disease in the USA12. In 1994-1998, the primary disease ratio of the total of dialysis patients was 39.5% for diabetes, %25.2 for hypertension/large vessel disease, 9.1% for glomerulonephritis, 3.8% for interstitial nephritis/pyelonephritis, 2.8% for cystic/hereditary/congenital diseases and 12.5% for uncertain etiology/missing cases. For renal Tx patients, these ratios were 16.1%, 4.2%, 14%, 4.5%, 13.8%, 42.6%, respectively12. 1.2.3. Treatment Modalities of ESRD Since 1990, the number of patients on haemodialysis has almost doubled and the number of patients with transplants has increased more than 90% in the USA12. Peritoneal dialysis has also increased, though this modality still accounts for only 13.5% of the ESRD population. In 1998, the number of dialysis patients was 243.524 in US and the number of point prevalent patients was 211.800 for haemodialysis, 25.300 for peritoneal dialysis, and 99.840 for transplantation. Growth in the prevalent dialysis population is a reflection both of growth in incident rates and of declining death rates. Projections for 2010 suggest that the number of both dialysis and transplant patients will reach levels double those in 1997, while the number of patients on the transplant waiting list will grow almost two and a half times during the same period12. In the EU countries, the incidence of ESRD due to diabetes, hypertension and renal vascular disease nearly doubled over 10 years in 1998-1999: it varied between countries from 10.2 to 39.3 pmp for diabetes, from 5.8 to 21.0 for hypertension and 1.0 to 15.5 for renal vascular disease11. The incidence of RRT in diabetic patients in Europe is only a quarter that of the US white population, which was 94.8 pmp in 199913. The cause of this geographic variation is probably multifactorial, including variation in the diabetes and/ or in the management of patients (secondary prevention) differences in mortality and also the persistence in some countries of limited access to RRT for these patients. While there is a wide range in the incidence of RRT for ESRD attributed to hypertension, from 5.8 pmp in Finland to 21 pmp in Norway, all European countries studied reported a much lower incidence than has been reported for the white population of Racial differences are evident across modalities in both incident and prevalent populations. Between 1994 and 1998 the greatest percentage increase in the number of haemodialysis patients has among the Native Americans and Asian populations, 11.2%, %8.5, respectively. Symptomatic ischemic heart disease was present in 38% and heart failure in 35% of the Canadians at first dialysis17. patients were more likely than white patients to be on haemodialysis12. In the EU during the 1995, the first RRT was HD in 80.8% (with a range from 61.4% in the UK to 97% in Portugal), was PD in 18.2% with a range from 2.7% in Portugal to 37.5% in the UK) and pre-emptive Tx in only 1.0%3. The mean percentage of dialysis patients undergoing a renal transplant during the year was 6.5% ranging from 2.8% in Greece to 23.3% in Ireland. In the USA, the number of first year deaths per 100 patient years in 1998, was 29.9 for all dialysis patients, 28.1 for HD, 27.8 for PD and 2.9 for Tx. Five-year survival was approximately %33. Five-year survival curves have shown that female patients live longer than males across most age groups, races, primary diagnosis, and modalities12. 1.2.4. Survival, Mortality and Causes of Death in ESRD Patients Despite advances in dialysis and transplantation, the prognosis of kidney failure remains bleak. The USRDS reported more than 60000 deaths of patients with ESRD, and an annual mortality rate of dialysis patients in excess of 20%13. Expected remaining lifetimes of patients treated by dialysis were shorter than the age matched general population, varying (depending on race and gender) from 7.1 to 11.5 years for patients aged 40 to 44 years, and from 2.7 to 3.9 years for patients aged 60 to 64 years. Atherosclerotic cardiovascular disease accounts for approximately half of the deaths in ESRD and contributes to the extraordinary high total annual mortality of 23% observed in such patients13. Incidence of myocardial infarction and stroke in the dialysis population is 5-to15-fold higher in ESRD15. The second principal cause of death is infections and diabetics have the highest rate of death. Morbidity of kidney failure is also high. The mean number of co-morbid conditions in dialysis patients is approximately 4 per patient. The mean number of hospital days per year is approximately 15, and self-reported quality of life is lower than that of general population13. Hypertension and diabetes are the complicating co-morbid conditions found most often in new patients, 73.9% and 64.4% respectively, followed by congestive heart failure (33.4%), and ischemic heart disease (24.6%) in the USA12. In the EU during the year 1995, the death rate among treated ESRD patients was 10.4% ranging from 5.4% in Ireland to 12.6% in Denmark3. In all fatal cases during 1995, the treatment modality was HD in 75.3% (ranging from 53.9 in the UK to 93.35 in Luxemburg), PD in 17.8% (from 0% in Luxemburg to 29% in Greece) and Tx in 6.9% (ranging from 6.9% in Ireland to 17.2% in Finland). The mean death rate was 13.4% for HD (ranging from 9.6% in Greece to 29.85 in Ireland) as compared to 20.7% for PD (ranging from 6.95 in Ireland to39.7% in Greece). The mean death rate for patients with transplants was 2.2% with values from 0.5% in Greece to 8.8% in Portugal. The 1995 incidence of death from ESRD was highest in the USA with 191 pmp, while it was115 in Japan and 64 in Canada, as compared to 67 in the EU. 1.2.5. Economic Costs of ESRD There is a clear, direct relationship between gross national product and availability of RRT. Dialysis treatment absorbs 0.7 to 1.8% of the health care budget in European countries, while the dialysis population represents 0.02 to 0.05% of the whole population2. Consequently, economic costs of ESRD are high; in 1999, more than 400000 Americans required dialysis therapy and/or transplantation for kidney failure, and Medicare medical expenditure for ESRD care was more than $11 billion (KEEP3). With the current rate of growth, there will be 172,667 incident and 661,330 prevalent patients by the year 2010. The growıng global burden of end stage renal dısease (ESRD)) Mexico27. Prevalence was largely dependent on the economic standing of individual countries. It was highest in Mexico, lowest in Pakistan. ESRD will increase from $ 12 billion to 28 billion over this time period 2. The costs of medication and patient care are also rising. Between 1994 and 1999 spending on recombinant human erythropoetin increased by 100% in the USA, the cost of intravenous (i.v.) iron supplementation increased by 50% and cost for i.v. calcitriol or other vitamin supplements increased by 200% USRDS8. Practically everywhere, public chronic HD is the most expensive modality, followed by private and limited care HD. The least expensive modalities are home HD and CAPD, whereas APD costs are in the same range as limited care HD. 1.3. Epidemiology Developing World of ESRD in The availability of dialysis and transplantation is quite variable in Africa28. There is only small number of patients on RRT in southern and sub-Saharan Africa. In comparison, treatment rates in North Africa are 30 to 186.5 pmp in countries with more established programs: Algeria 78.5, Egypt 129.3, Libya 30, Morocco 55.6, Tunisia 186.5 pmp. 1.3.2. Etiology of ESRD Etiology of ESRD in the developing world also varies between geographic regions and economic conditions. Glomerulonephritis is the most common cause of ESRD in India and Pakistan27. It is similar in countries within the same World Bank Low- economy category and less prominent in countries in the Medium-economy category, ranging from about 11% in Egypt and Argentina to 28% in Saudi Arabia. Proliferative glomerulonephritis constituted the major bulk of primary etiology in contrast to its remarkably low frequency in the developed world. the 1.3.1. Incidence and Prevalence of ESRD The exact number of patients with chronic renal failure requiring RRT in the developing world is not known. Unlike the developed world, most developing countries lack renal registries. Therefore, the exact incidence and prevalence of ESRD in the population, its burden on the health care system, and the outcome of these patients are not known. The reported annual incidence of patients with ESRD varies widely, from as low as 4 pmp in Bolivia to as high as 254 pmp in Puerto Rico18,19. Incidence rates of 52 pmp and 200 pmp were reported in Turkey and Egypt, respectively (20,21). In the rest of the world, the estimated incidence ranges from 100 to 200 pmp22. In India, an estimated incidence of ESRD of 100 pmp, approximately 100,000 patients develop ESRD each year23. The reasons for such wide variations are likely to be differential racial factors, environmental conditions, the criteria used for diagnosis, and the availability of sophisticated dialysis programs23. The number of patients accepted by dialysis programs is about 80 pmp in Egypt21, 20 pmp in Malaysia24, 3 to 5 pmp in India25 and China26. Focal and segmental glomerulosclerosis is the second most common lesion in Africa. In contrast, Ig A nephropathy is uncommon in Africa. On the contrary, the prevalence of Ig A nephropathy is high in Thailand. Amyloidosis was reported as a fairly common glomerulopathy with a prevalence ranging from 6.1% to 10.3% of all glomerular lesions27. İt may be attributed to chronic infection as tuberculosis in India29, schistosomiasis in Egypt, or to familial Mediterranean fever in North Africa30. Interstitial nephritis accounted for 2.4% to 20% of reported ESRD: the highest prevalence being encountered in India and Pakistan27 In the developing world, diabetic nephropathy that is increasing over the years, constitutes more than 25% of the dialysis population. It appears to be higher in Latin America and India than in Africa. The reported number of haemodialysis units in other developing countries was a gross average of 5.2 pmp27. There was an additional small pool of units providing only peritoneal dialysis, the size of which averaged 20% that of the haemodialysis pool. In the absence of adequate dialysis facilities, renal transplantation remains the only hope of survival for ESRD patients in developing countries. In contrast to developed countries, the underdeveloped nations have no organized cadaver transplant program. Utilization of cadaveric kidneys is poorly organized in Asian countries. Egypt to 29.9% in Thailand. Hypertensive nephrosclerosis accounted for 13% to 21% of reported ESRD27. But the range is much wider in other reports, spanning between 4% in the Sudan (29) and 43% in Nigeria31. The mean age of ESRD patients requiring dialysis in most developing countries is much lower, 32 to 42 years, than that in the developed world, 60 to 63 years32-35. Among the reasons for this difference are the delay in detecting renal disease and the failure to institute controlling and preventive measures in patients with progressive renal failure, both of which result in faster deterioration of renal function and progression to ESRD23. Patients aged under age 18 years constitute about 3% in Egypt and Thailand, 12% in Mexico and South Africa. There is a significant male preponderance in developing countries that varying from about 52% in Thailand to 68% in Egypt and up to 80% in India. This may be partly explained by the increased exposure of men to noxious environmental factors27. Considering the reported incidence of ESRD in different countries, gross diversity can be seen in the priority of offering transplantation as the preferred treatment modality. Sources of kidney donation for transplantation are mainly living donors ranged from almost 0% in Egypt to 28% in South Africa. In contrast, in some developing countries, cadaver donors account for approximately 50% to 60% of transplants such as Argentina and Thailand36. About 2500 to 3000 renal transplants are performed in India each year37. Of the 3000 transplants, only about 100 are of cadaveric origin. Living unrelated donors account for 50% to 60% of transplants in India, %20 in Egypt, and the rest are from living related donors27,37. 1.3.3. Treatment Modalities of ESRD CAPD is becoming the preferred modality of dialysis in countries with limited resources. The procedure does not require expensive equipment, and after the initial period, the patient no longer requires regular visits to a dialysis centre. These qualities should make CAPD the ideal modality for ESRD patients However, in developing countries23. utilization of CAPD varies greatly among developing countries. In some South American countries like Mexico, Venezuela, and Brazil, 50% to 90% of dialysis patients are on CAPD. Its use in other developing countries such as Chine, Thailand, Saudi Arabia, Egypt, Tunisia, Argentina and India is low23,27. In contrast to that in the developed world, the cost of CAPD is two times higher than that of haemodialysis in developing countries. The main reason for this disparity is the lack of indigenous facilities to manufacture peritoneal dialysis fluid27 1.3.4. Survival, Mortality and Causes of Death Intermittent peritoneal dialysis is seldom used as a method of long-term life support with an average annual survival of less than %10. The mean annual survival on CAPD is 62% ranged from 30% in Egypt to >90% in Tunisia and that on haemodialysis is 73.4% ranged from approximately 10% in India to 90% in Egypt and Tunisia35. The principal causes of death are cardiovascular (43% to 51%) and infection (16% to 23%). The incidence of tuberculosis in dialysis patients has been reported to vary between 4% and 15% in tropical countries38. The average prevalence of hepatitis B and HCV in a survey was found as 9.4% and 32.8%, respectively27. HIV infection has not yet been reported to be a major problem in the dialysis population in developing countries39. 3. 4. 5. 6. Infections complicate the course in 50% to 75% of transplant recipients in developing countries, with mortality ranging from 20% to 60%40-42. In comparison, developed countries have an incidence of infection of about 40%, and advances in diagnosis, prevention, and therapy have reduced mortality to less than 5%43,44. 7. 8. 1.3.5. Economic Cost of ESRD 9. The items of typical annual direct dialysis cost per capita are essentially the same between countries. It is noteworthy that peritoneal dialysis is the more expensive modality in countries that still import the dialysis solutions. For example, costs of dialysis modalities are approximately $ 22,644 for haemodialysis, 22,350$ for peritoneal dialysis in Turkey20. CAPD is also currently an expensive treatment modality in Africa27. On the other hand, countries that could overcome this barrier such as Mexico and South Africa are able to provide this modality at a much lower cost26. 10. 11. 12. 13. 14. In conclusion, the rate of treated ESRD continues to increase globally and the economic burden of RRT is increasing in both developed and developing countries. To reduce the cost of peritoneal dialysis fluids and to increase the rate of transplantation seems the best way to increase the availability of RRT for ESRD patients in the developing world. 15. 16. 17. A 17-year-old female patient presented with headache and swelling of the left cheek over a one-year period. She also complained of recent onset facial tingling, and rare attacks of seeing lightning flashes. She denied any nasal obstruction, and nasal or postnasal discharge. There was no family history of bone disease, genetic abnormalities, or endocrinopathies. FD is a benign, localized bone disorder of unknown etiology. Pathologically, the disease is characterized by a slowly progressive replacement of normal medullary bone by fibrotic tissue and disorganized bony trabeculae1. The bony cortex may be thinned, but remains intact, maintaining a smooth contour2. Because of the slow progression of FD, it may take years to become symptomatic1. The presenting findings are ptosis, exophthalmos, diminution of visual acuity, and cosmetic deformity1-3. The disorder occurs during childhood and becomes inactive when skeletal growth is completed1. The craniofacial skeleton is a common site of occurrence. The disease has three different variants1-5: type 1, characterized by single or multiple lesions in the same bone (monostotic); type 2, characterized by multiple lesions involving two or more bones (polyostotic); and type 3, characterized by dissemination of pathological bone changes connected to Albright’s syndrome (precocious puberty, cutaneous pigmented patches over the involved bones). Monostotic FD accounts for 70% of cases and tends to become evident in late childhood. Polyostotic FD accounts for about 30% of cases and usually becomes evident earlier as a result of multiple bone involvement. Craniofacial bones are affected 10-30% in monostotic form, and 50-100% in polyostotic form depending on the severity. Craniofacial FD is most frequently located in the mandible and maxilla. The paranasal sinuses are rarely involved. With craniofacial involvement, symptoms relate to bony overgrowth1. Diagnosis mainly depends on the radiographic findings. Craniofacial FD has been classified into 3 radiographic patterns: pagetoid, sclerotic, and cystlike1,5,6. The pagetoid form (56%) is with remarkable bone expansion and alternating zones of radiodensity and radiolucency. This type more freqently appears in patients over 30 years of age. The sclerotic type (23%) demonstrates a homogeneous density, again accompanied by bone expansion, whereas the cyst like pattern (21%) is usually a single, well-defined radiolucent lesion, oval in shape, edged by a thin, sclerotic margin clinically resembling an eggshell. Both are frequently seen in children or younger adults who have had symptoms for less than 3 years. CT provides greater detail of the extent of FD than does conventional radiography. CT findings are pathognomonic, and also aid in monitoring the progress of the disease. On CT, FD has a characteristic homogeneous ground-glass appearance1. Treatment is recommended in symptomatic cases, and the only treatment modality is surgical debulking. As FD is likely to become quiescent, surgical intervention should be as conservative as possible, and should be guided by the radiographic findings. The patient should be informed of the unpredictability of FD and the tendecy for recurrence. Prognosis depends on the progression of the disease and is good in nearly all cases.
